  1. LAW ENFORCEMENT
  2. The Contractor will provide a full-time off-duty uniformed Police officer for traffic control as needed, as described in General Notes and Specification Data to Item 502.

  4. All materials stored on the site that the contractor intends to get paid for as material on hand will be accessible for review at any time, in accordance withTXDOT Specification 9.7.
  5. Failure to supply all invoices with requests for payments of Materials-on-Hand will be grounds for denial of payment.
  6. No obstruction of sight distances or views shall be permitted on any active roadway.
  7. Ensure that all stockpiled materials are kept out of the clear zones of anactive roadway.
  8. SUBCONTRACTORS:
  9. The Contractor shall not subcontract any portion of the contract without the approval of the Engineer. No subcontractor will, in any case, relieve the prime contractor of his responsibility under the contract.

  8. SAFETY AND HEALTH:
  9. It is the contractual obligation of the Contractor to comply with the State and Federal construction safety standards. Further, it is the Contractor’s responsibility for seeing that the subcontractors also comply with safety standards.
  10. To document the Contractor’s accident program , the Contractor must answer the following questions:
  11. Does the Contractor have a defined safety program?
  12. Has responsibility for safety been assigned to a specific company official?
  13. Does the Contractor make frequent project safety inspections? Does this include subcontractor operations?
  14. Does the Contractor’s management support the safety program?
  15. Does the Contractor’s safety program include discussion with employees to promote safe conditions and practices in their work?
  16. Are periodic safety meetings held with employees?
  17. Are all job-related injuries and illnesses investigated, recorded and reported by the Contractor?
  18. OSHA Compliance. Does the contractor keep informed on Government safety regulations and standards?
  19. ENVIRONMENTAL:
  20. There will be a separate meeting scheduled and held with contractor and USACE to discuss the compliance with the 404 Permit requirements for the corridor. The meeting is scheduled for ______AM/PM at [Insert location of meeting]______

  34. A separate meeting to discuss schedule submittals, reviews, deadlines, and updates will be held at a later date.
  35. The Contractor is responsible for the damage to underground utilities located within the Right of Way. Reminder to review Special Provision SP-0.02.0 including submittals prior to construction operations in the area of existing utilities.
  36. Materials letter designating sources of all materials proposed to be used on project. STL will be the Materials Testing firm supporting the CM.
  37. Periodic utility coordination meetings will be held as the project progresses.
