Reason for coming: Concerns about speech sound development; child difficult to understand

1. multiple speech sound error patterns (“phonological processes”)

2. child’s ability to be understood by others significantly affected

Details about the Major Areas of Concern:

Speech

·  Formal Measures (Goldman-Fristoe Test of Articulation – 2)

o  Low-average/borderline score when compared with same-age children

o  Consistent error patterns observed:

§  “stopping” = p/f, d/s, s/ “sh” (these patterns typically extinguished by ~3 years)

§  “deaffrication” = t/ “ch”, d/ “j” (these patterns typically extinguished by 3 – 3;6 years

§  “gliding” – w/r , w/l (these patterns may continue past age 5 years)

·  Conversation

o  Errors consistent with those noted above

o  Child could be understood ~ 65- 70% of the time

·  Stimulability (Ease of teaching child to correctly produce sounds in error):

o  /f/, /s/, / “sh”/ - isolation level with prompts, difficulty at syllable level and above

o  /l/ - very difficult

o  Within normal limits and sometimes above average comprehension and expression of language.

·  Cooperative and hardworking

Structure and Function of Oral Musculature

·  Considered within normal limits and adequate for the production of speech

Recommendations:

1.  CL is a good candidate for speech therapy to “extinguish” the patterns of “stopping” and “deaffrication.” He will be placed on waitlist for treatment here at the clinic. Other private referrals will be made in the report.

2.  Parents to provide good speech models. Focus on keeping the conversation going rather than on producing sounds correctly. Ask the child to imitate infrequently. Any appropriate parent “articles” will be enclosed with the report.
