Definition of Satellite Clinic and Process for Including in RI-CSI (Draft 8 6 14 )
Satellite Clinic Definition: A satellite clinic is a primary care practice site that is affiliated with a larger CSI RI primary care entity (“the parent”) but stands alone physically. The satellite site shares providers who also work at the “parent entity”, patients may be seen in either location. Satellite clinics share the same policies; procedures; and infrastructure of the “parent site” including electronic health record.

Multisite Practice Definition: – the practice represents a discrete practice site with dedicated staff and their own panel of patients.
Process for Satellite Clinic to become part of the CSI program
Should a current CSI-RI site wish to open a satellite location the following standards must be met.
- Parent organization notifies CSI-RI that a satellite clinic would like to be added to the CSI program and completes the abbreviated application process.
- CSI reviews the application and provides report to Executive Committee
- CSI will notify the health plans of the Executive Committee decision and the health plans will work with the parent organization and satellite clinic site to complete any credentialing requirements and signing of contracts, if applicable.
- Depending on the geographic location, compacts and may need to be satellite specific.
- The majority of providers practice between both the parent and the satellite site. In turn, providers will continue to be attributed under the same NPI and TIN information. The stage of developmental contract and concurrent PMPM rate of the parent site applies to any patients seen at either parent or satellite sites.
- Measurement continues at the site level, quality; CAHPS data will be required for the satellite. For quality and customer experience, performance data will be aggregated across both the satellite and the parent organization.
Should a satellite begin to host a majority of providers at the satellite locationt site, they would be held to the following standards as an additional practice site (Multi-site) :
- Six months prior to the end of the current contract year, the satellite site and parent organization must notify both project management and participating health plans of these changes
- NCQA level 3 recognition must be achieved at the level of the satellite site
- The satellite site must identify which providers will be located at which sites, via an update to project management’s master provider file
- The satellite site must contract with participating health plans at the same developmental stage as the parent site, at the start of the next contract period.
- Satellite site will then be held to contractual performance standards and metrics corresponding to the proper developmental stage in the developmental contract
- Clinical Quality measures must be generated, for the satellite site, upon notice of change. If no baseline data, with proper look back period, are available for satellite site, then sites must perform towards targets for the current contract year. They will not be eligible for success by improvement methods.
- Patient Experience measures must be generated for the new practice.If no baseline data, with proper look back period, are available for satellite site, then sites must perform towards targets for the current contract year. They will not be eligible for success by improvement methods.
- Satellite site will now be aggregated with same cohort as parent site.
Going forward health plans would like to have a master contract for multisite practices with pmpm’s rolled up to one payment for the practices on a quarterly basis. CSI will determine the blended rate based on each practices performance against targets; level of the developmental contract and attribution associated with each site. Differential distribution of performance incentives to practice sites within a multisite practice will be left to the discretion of the corporate entity representing the multisite practice.
