POSITION TITLE:Intake and Assessment Practitioner

PURPOSE
The positionprovides comprehensive intake and post-ventionservices with potential clients of Centacare North Queensland. Through service activities such as intake, needs and safety assessments, supported referrals and inclusive service planning, the Intake and Assessment Practitioner will play a key role in client engagement as well as post-service evaluation of client outcomes.
KEY RESPONSIBILITIES
- Coordinate and deliverIntake and assessment services at Centacare including pre-service safety screening and post-service follow up with clients in line with organisational protocols and funding body requirements
- Provide information and resources to clients in a supportive and proactive manner, including community information sessions when required
- Coordination of community resources available to staff and clients
- Manage duty office calls as part of the roster system
- Demonstrate a commitment to the mission of Centacare North Queensland, as well as the organisational and funding body goals.
- Comply with legislative (including Workplace Health and Safety, Privacy and Equal Employment Opportunity) and Quality Management System requirements as well as participate in program and business planning.
- Ensure intake and assessment services respond to organisational and funding body expectations, including services being accessible, responsive and respectful
- Other duties as delegated by the Family Relationship Services Manager.
KEY PERFORMANCE INDICATORS
- All agency community resources are managed effectively using appropriate systems.
- Effective and efficient coordination of Intake and assessment will occur including liaison with relevant client services staff in a timely and accurate manner.
- Clients are effectively engaged with programs, supports and resources according to identified needs, which is documented appropriately and in a timely manner
- Clients are supported through their service engagement in a planned and responsive manner, including post-vention follow up to assess outcomes and future needs.
- Tasks as delegated by the Family Relationship Services Manager are completed in a timely, efficient and effective manner.
SELECTION CRITERIA
Mandatory Selection Criteria
MSC1:Possession of a relevant degree or qualifications in the Social Sciences and experience in the social services sector.
MSC2:Ability to possess a Positive Notice for Child Related Employment (Blue Card and a yellow card exemption) fromthe Public Safety Business Agency (PSBA).

Key Selection Criteria
KSC1:Demonstrated knowledge of relevant legislation, funding body requirements and service delivery related to Centacare and its programs
KSC2:Highly developed interpersonal and communication skills including the ability to form effective working relationships with internal and external stakeholders
KSC3:Demonstrated experience or understanding of intake and assessment processes and engaging with complex and diverse clientele
KSC4:Demonstrated experience in the delivery of information and resources to small groups, the provision of written documentation such as reports, articles and correspondence.
KSC5:Highly developed organisational, time management and administrative skills along with competency in a range of computer applications including word, excel, publisher.
Desirable Selection Criteria
DSC1: Willingness and ability to work outside normal office hours as determined by the Family Relationship Services Manager
DSC2:Experience working as part of a large and multi-disciplinary team.
