Hours / 35 hours per week, Monday to Friday between 9am and 4.30pm Fixed Term Contract to 31st March 2016
Wage/ salary / £ 17,852
Reports to / Head of Health & Wellbeing
Responsible for / Providing administrative and operational support to the Hospital Discharge Support Team and service users.

Travel expenses at 40p per mile are paid for the use of your car insured for business purposes in the course of your work. It does not include travel from home to normal place of work.

Scope:

Main purpose of the post:

·Providing an administrative and operational support service.

·  Work as a member of the Hospital Discharge & Community Support Scheme (HDS) with the Head of Health & Wellbeing and project workers.

· The base for the role will be the Hitchin Office, Lister and QE2 with occasional travel required to our other countywide offices. (Limited parking on rota basis at Hitchin Office)

· To manage telephone calls, referrals, assessments and communication

· To update and maintain and the Age UK Hertfordshire IT contact management system (Charity Log), Enquiry Tracker databases including SOUL and spreadsheets

· To input data, update data and produce reports for monitoring and evaluation purposes

· To assist with the scheduling of volunteers

Responsibilities/ Duties / tasks

· Carry out day-to-day HDS administration tasks including: word processing, emails, mail outs, filing, photocopying, and handling telephone enquiries

· To provide occasional service delivery/operational support as required by the Head of Health & Wellbeing

· Manage communication and transfer calls as appropriate or take messages ensuring they are recorded via the appropriate systems

· Type correspondence as required by the Head of Health & Wellbeing

· Provide administrative support to Head of Health & Wellbeing and HDS coordinators

· Provide cover in the absence of the coordinators as requested by the Head of Health & Wellbeing

· Provide assistance with signposting and referrals

· Refer service users internally to other schemes and aid with scheduling of visits

· Act as a communication network within the organisation for HDS Lister and QE2

·  Liaise with hospital and A&E staff as required by the Head of Health & Wellbeing

·  Visit service users on wards and A&E departments prior to discharge to undertake initial assessments

· Responsible for office filing and storage systems which will enable HDS to function in an efficient way

· Keep notice boards and publicity areas relating to HDS up to date and tidy

· Responsible for the prompt receipt and postage of mail

· Support HDS volunteers with the guidance of the Head of Health & Wellbeing

· Provide practical support to volunteers and staff

· Prepare and distribute agendas and minutes of meetings as for the HDS team as required

· Deliver food packs to clients as required

· Maintain client and staff confidentiality at all times

· Awareness of Health and Safety policies and procedures

· Attend training courses when a need is identified
