Community Activist Award
The American Legacy Foundation (Legacy) is a national public health foundation that works to keep young people from smoking and to help all smokers quit. Along with broad-based national campaigns aimed at these twin goals of prevention and cessation, the foundation has been devoted to community service through an extensive grants program, sponsorship support, and working with public health and tobacco control advocates at the state and local level.
Each year, Legacy pays tribute to individuals who demonstrate leadership and courage in working toward a tobacco-free world by honoring superb individuals at the annual Legacy Honors fundraising event. The fourth annual Honors event will take place on March 12, 2007 in New York City.
In the spirit of Legacy’s community awareness, the foundation is now accepting nominations for the prestigious Community Activist Award, one of the honors to be bestowed that evening.
Past Community Activist winners include
· Gerry RainingBird (Chippewa Cree), a leader for tobacco control in Native American communities who incorporated traditional and sacred used of tobacco to develop resistance to its commercial use;
· Sherri Watson-Hyde, an advocate for tobacco control programs that engage and empower minority communities who has worked to educate and raise awareness about tobacco use; and
· Lourdes Baezconde-Garbanati, a researcher focused on community tobacco prevention and community cancer control among medically underserved communities, with special emphasis on Hispanic/Latino population groups.
In the spirit of excellence and commitment of the past winners, Legacy is searching for tobacco control leaders who work tirelessly in the cause of tobacco prevention and control in their communities. Nominees should have a minimum of one year of active work on tobacco control issues, especially those issues that support the goals and mission of American Legacy Foundation (www.americanlegacy.org). In addition, the ideal candidate will have championed or spearheaded innovative or promising tobacco activities and be recognized and respected as a leader in his or her community.
A three member committee comprised of tobacco control leaders will select the award winner with support from American Legacy Foundation staff.

1.) Please provide contact information for the nominee and, if different, the individual submitting the nomination. Type or print clearly.
2.) Please provide a short narrative about the nominee that addresses the following questions:
a) Describe the nominee’s current and past roles in the tobacco control community.
b) Explain the length of time the nominee has been active in tobacco control and prevention issues.
c) Provide details about how the nominee’s work demonstrates innovation and/or progress in tobacco control.
d) Legacy’s mission is to build a world where young people reject tobacco and anyone can quit. Legacy’s goals are to (1) arm all young people with the knowledge and tools to reject tobacco and (2) eliminate disparities in access to tobacco cessation services and prevention. How have the activities and leadership of this nominee supported Legacy’s mission and goals?
