AUTHORIZATION AND AGREEMENT FOR ELECTRONIC SERVICES

I hereby authorize Charles Eastwood d.b.a. Locksmith Charley or any of his assigned agents to perform an electronic procedure on the Engine Control Module that I will supply to him. The electronic procedure is designed to return the Engine Control Module to its original “new” state and to have the ability to learn new transponder keys without an existing Pre-Registered Master Transponder Key.

I also agree that I am the owner of the vehicle or I will correctly determine the owner of the vehicle and will gain his/her permission to have this procedure done. I will collect his/her full name, address, telephone number, License plate number, Vehicle Identification Number (VIN), year, make, and model of the vehicle for which the Engine Control Module is to be used.

Customer understands that Toyota / Lexus manufacturing company has chosen to be inconsistent with the computers that they put in certain models and that if the flashing procedure designated for that vehicle fails to work properly customer’s only recourse is to return the ECM to Locksmith Charley for a subsequent flashing procedure. Customer agrees that under no circumstances will customer charge back or dispute credit card charges for this service.

Customer understands and agrees that the vehicle’s battery is to be disconnected prior to removing the ECM from the vehicle and is to stay disconnected from the vehicle until after the ECM is completely re-installed into the vehicle.

Charles Eastwood d.b.a. Locksmith Charley does not warrant any circuitry in the Engine Control Module supplied to us. No liabilities for any use of the modified Engine Control Module are assumed whatsoever for any costs, claims, or damages filed for or caused by correct or incorrect use of the modified Engine Control Module.

Charles Eastwood d.b.a. Locksmith Charley cannot be and is not responsible for any type of damage, intentional or otherwise, caused by the use of the modified Engine Control Module. Charles Eastwood d.b.a. Locksmith Charley is not responsible for any type of harm, whether physical or otherwise, to the vehicle owner, car dealer, parts supplier, locksmith, or their customers due to correct or incorrect use of the modified Engine Control Module.
