What is a Community Transformation Grant?

Community Transformation Grants (CTG) is a program initiated by the Centers for Disease Control and Prevention (CDC) and funded through the Prevention and Public Health Fund (PPHF). A CTG aims to improve community health through community partnerships and community-based interventions. It is expected that the implementation of a CTG in your community will improve the health of every 4 out of 10 of your residents. What is the purpose of the Prevention and Public Health Fund?

The PPHF was established by the Affordable Care Act (ACA) to improve health outcomes in public health settings. The Fund, which is the nation’s largest single investment in prevention, using evidence-based and innovative partnership approaches to improve the health of Americans, provides more than $14.5 billion in mandatory appropriations over 10 years to improve health and prevent chronic illnesses by expanding preventive care and supporting proven community-based programs that reduce obesity, tobacco use and other preventable conditions. More specifically, the PPHF has invested in a multitude of evidence-based services including community and clinical prevention initiatives; public health infrastructure; immunizations and screenings; tobacco prevention; social and emotional wellness (such as facilitating the early identification of mental health needs and access to quality services); and public health workforce and training.
