If these services were being denied, they should be receiving a letter with the appeal process, however the client did not receive such a letter.
Community Health Workers are all going through training right now. They have one more week of training to get certified. New Community Health Workers at Good Shepherd are covering the emergency room on the weekends to keep the volume of visits down.
Aimee is now getting the EDDIE report.
Nora reported that there were 150 people and 11 booths at the Health Fair. Nora was pleased with the turnout and wants to thank all of the partners who came out to support. The majority of the attendees were OHP clients, if not all of the attendees.
Heidi reports that Project Connect in Hermiston was well attended with 125 participants and 50 Veterans.
Estella reports on the education summit held last Friday. There were 450 individuals who attended. A new sanctuary model, that communities may adopt, was introduced at the summit. This model takes ACES one step further. Kevin asks of those who attended the summit, if this model is something we can achieve.
EOCCO would be willing to underwrite the expense of the training. The goal is to get the County Gov. and agencies to have a common understanding of the impact of trauma and the ability to respond in a trauma informed fashion.
If Assisters have problems while working with clients, Oliver is the person to help with those issues.
Heidi is working on a list of Certified Assisters and a schedule as to when they are available to assist in OHP enrollment.

AGENDA ITEM / RECOMMENDATIONS/ACTIONS
Community Health Improvement plan
6. Incentive Measure Committee Update
7. Community Health Assessment FY16 Update
8. Violence Prevention Update – / MODA will be presenting at the Incentive Measure Committee meeting today.
The Health Assessments have gone out and the community is now receiving them. To be valid, Morrow County needs 324 surveys returned and as of Wed they have 319. Umatilla County needs 374 surveys and as of Tuesday they have 279. Union County needs 334 surveys and they are already at 373.
Our Hispanic population is struggling to read and understand the assessment. Rod asks which entities are willing to do these assessments as we need to have these surveys completed by our Spanish population. Nora states that her agency is willing in Hermiston, Irrigon, Boardman, and Milton Freewater. Nora will begin to take these surveys out right now.
The Health Departments are meeting again next week to coordinate together on the Childrens Health Assessment. The assessment will happen, just unsure of the timing at this point.
To report in 3 Counties, the cost to get this going is roughly $26,200.00. 60% is needed up front to get the project rolling. Data on the children was not available the last time we did this process. More information about children is definitely needed. Roughly $12,000.00 more is needed so donations are welcome.
Good Shepheard and St. Anthony are the signer for the adult contract and have put down the 60% for the assessment to move along. If there is anyone else who wants to contribute, you may. Good Shepheard and St. Anthony may also do the same for the children’s as long as the partners also have investment.
The candidate for the coordinator position has accepted, however has asked to not be identified until they have given notice to employer. /
ACTION: Nora will begin to take the Spanish Assessment out to clients right now. Any other agencies that will be able to do the surveys, please contact Rod.
AGENDA ITEM / RECOMMENDATIONS/ACTIONS
Other Activities
9. Report out on Transformation Grant Activities
10. Report on Regional CAC Meeting in Ontario
11. Statewide CCO Summit Nov 17th in Portland
12. Good of the Order, agenda
Items, and set a date for the next meeting: / Kevin reports that there is a very short deadline to ask for money to continue funding for transformation grants.
Kevin feels that if agencies work together to put in a unified application, rather than competing against one another, that there is a better chance in receiving funding.
There is a commitment for the funding to come out in February if those applications are on time. If there is funding needed before then, contact Kevin.
The money allocated to the Community Advisory Council will be on the same timeline.
In the metrics, there was $750K in existing grants along with the same amount for new grants. Since that time they learned more money was available, limited to EOCCO and the $800K came from other CCO’s that didn’t make their metrics.
There is a real advantage to agencies coordinating their efforts rather than competing. This money is not ‘use it or lose it’, it will be there and available.
Registration for the 2015 Coordinated Care Model Summit, November 16 & 17, is now open. Register by November 12th for this event. More information available online.
Community health worker training will be done next week. Whitney is teaching the class and next month Carol will be teaching the class. If there are others in the agency who need the training, talk to Juli as she is getting it coordinated. It’s a 5 or 6 week training, 50 hours online and 30 hours live.
Next meeting is November 13, 2015 in Pendleton at St Anthony Hospital. / ACTION:
ACTION: Kevin will email the correct CAC application as the processed has changed.
