Stratis Health has been selected for a Special Innovation Project to assist PAC providers, hospitals (acute care settings), physicians, community organizations, and other stakeholders in the community to identify best practices and solve barriers to PAC provider HIT adoption to support health information exchange at times of transitions in care and shared care.

Key collaborators will be the Minnesota Hospital Association, Care Providers of Minnesota, Aging Services of Minnesota, the Minnesota Department of Health, Minnesota Alliance for Patient Safety, CHIC (Community Health Information Collaborative), and the LeadingAge/Center for Aging Services Technology.

Long Term Post Acute Care (LTPAC) Facilities

Individual post acute care providers in Minnesota that qualify and are selected for the HITPAC program will be able to receive no-cost HIT consulting services. We offer an extraordinary opportunity to help you with your EHR implementation and optimization. You will receive help with the following:

·  Technical assistance

·  Identify barriers and best practices to advance the use of Health Information Technology in NHs

·  Use of patient assessment content (e.g., MDS content) to support transitions in and instances of shared care on behalf of Medicare beneficiaries receiving post acute care services

·  Work with you on Health IT standards that support the interoperable exchange of patient assessment content and the alignment of these standards with requirements in the CMS EHR Incentive Program (Meaningful Use). We particularly are interested in the exchange of information between nursing homes and hospitals

·  Educate about the opportunities available to improve the medication management process using HIT and HIE activities (e.g., electronic prescribing, medication reconciliation, and/or electronic medication administration record)

·  Assist with redesigning LTPAC facilities’ workflow and organizational culture to incorporate HIT into daily operations

·  Conduct at least 1 site visit to each nursing home per community to educate staff

·  Provide educational sessions on a quarterly basis

·  Strategies for implementation and use of data

·  Facilitation of members agreeing upon data elements

·  Training on workflow re-design methods


Acute Care Facilities - Hospitals

Area hospitals in Minnesota that qualify and are selected for the HITPAC program will be able to receive no-cost HIT consulting services to help you with your transitions of care implementation for your EHR. You will receive help with the following:

·  Work with you on Health IT standards that support the interoperable exchange of patient assessment content and the alignment of these standards with requirements in the CMS EHR Incentive Program (Meaningful Use)

·  Stage 2 Meaningful Use Hospital Core Measure 12 requires more than 50% of transitions of care and referrals with 10% sent electronically and at least one sent to a recipient with a different EHR vendor. The HITPAC program will help you to meet this and other Stage 2 measures around e-prescribing.

·  Assist with incorporating HIE into daily operations

·  Provide educational sessions on a quarterly basis

·  Technical assistance

·  Facilitation of members agreeing upon data elements

·  Training on workflow re-design methods
