Community Awards Celebration
Nomination Information
A time to honor and encourage community involvement is the hallmark of nearly every successful community. To that end, the Staples Motley Area Community Foundation solicits your nominations for the following Awards:
- Outstanding Citizen
- Outstanding Group or Organization
- Outstanding Business
- Outstanding Youth
- The Duane R. Lund Award for Lifetime Achievement
- Chairman’s Award
The Chairman’s Award category can be suggested by the person submitting the nomination. Possibilities might include: Outstanding Volunteer, Senior Citizen, Educator, Police Officer, Firefighter, Medical Person, Farmer, Child Care Provider, etc., anything that does not fit into the other four categories.
The Community Awards Celebration is scheduled forOctober 12, 2017. Nominations will be accepted through July 15, 2017.
Nomination letters should include the following:
- Contact information (name, address, phone number and email address) of the individual or organization making the nomination.
- Contact information of the individual, group, business or organization being nominated.
- A response to the following question: How has the individual or group contributed to the well-being of the Staples/Motley Community? Please be as complete as possible in answering the question with as many examples as you can list.

The Staples Motley Area Community Foundation is a component fund of the Initiative Foundation. Its mission is to develop and strengthen community resources, and to enhance the quality of life of individuals, families and organizations in and around the Staples community. Its focus is to strengthen youth and families, improve community appearance, and increase community involvement and a sense of belonging.
