REHABILITATION FOLLOWING SHOULDER ARTHROPLASTY
(TOTAL SHOULDER REPLACEMENT)

Phase 1 (0-6 Weeks after surgery)
Goals:
- Diminish joint swelling and pain
- Protect the repair. Allow the subscapularis to heal
- Prevent shoulder stiffness
Activity:
- Apply cold to the shoulder 10 to 15 minutes- 4 to 5 times each day or more initially after surgery.
- Remove the sling several times per day and with your arm relaxed down at your side move your fingers, wrist and elbow. Begin pendulum exercises several times per day.
- To wash under your arm- bend forward at the waist with your arm relaxed hanging down. This position will allow you to wash under your arm.
NO EXTERNAL ROTATION PAST 0 DEGREES FOR 6 WEEKS
NO ABDUCTION PAST 45 DEGREES FOR 6 WEEKS.
YOU WILL BE IN THE SLING EXCEPT TO COMPLETE YOUR EXERCISE PROGRAM AND BATHE FOR 6 WEEKS.
Physical Therapy (Weeks one through six)
- Lying on your back you will start to reach your arm up toward the ceiling with assistance from the therapist or using your other arm to help.
- While lying down the therapist willdo move your arm to prevent tightness from developing at the shoulder girdle.
- Shoulder shrugs and retraction
- Active and assisted shoulder flexion in standing will be started as comfort allows.
- Pulleys may be started as comfort and safety allow with permission from your therapist.
Phase 2 Weeks 6 to 8
Goals:
- Protect the shoulder from overstress
- Full active ROM and control
- Increase strength of shoulder
Activity:
- Full active and passive ROM in all directions
- Begin gentle strengthening exercises with theraband or light weights under the direction of your therapist
Precautions:
- No heavy lifting or manual labor
- Avoid having the arm pulled forcefully behind you
- Do not hang your body weight from your arms.
PHASE 3 Eight weeks and beyond
Goals:
- Protect the shoulder repair
- Regain full motion at the shoulder
- Progress in strengthening program
Activity:
- You may use the arm in a normal fashion in daily activities.
Precautions:
- No heavy lifting or manual labor until cleared by the surgeon
- No lifting weight above your head and allowing the weight to go behind your head. (Keep the weights in front of you where you can see them)
- Avoid having the arm forced behind your back.
Exercises:
- Standing external rotation at doorway
- Wall Climb
- Reaching overhead
- Supine cross arm stretch
- Sidelying external rotation
- Reaching behind our back
- Theraband and gentle PRE exercises.
