WELCOME TO LAVANT ROAD SURGERY

We are delighted to welcome you to the surgery, where we aim to provide a high standard of personal health care to all our patients. We are happy for you to consult any member of the practice, but it is to your advantage to have continuity of care from one doctor.

SURGERY HOURS

The surgery is open core hours from

8.00am – 6.30pm Monday to Friday.

We also run an evening ‘Commuter Clinic’ service on Monday evening with the last appointment at 7.30pm

and a morning ‘Commuter Clinic’ on Tuesday and Wednesday mornings from 7.30 am.

The Commuter clinic is a service for those patients who find it difficult to attend in core hours, e.g. those who work out of the area. Appointments must be booked in advance.

Please note the surgery only accepts urgent telephone calls after 6.00pm

WHEN THE SURGERY IS CLOSED

If you need urgent medical attention out of hours (evenings, weekends and bank holidays) there is an emergency On-call service provided by Harmoni, a company contracted by Coastal West Sussex Clinical commissioning Group (CCG).

This is for emergencies only,

for problems that cannot wait until the surgery is next open.

HOW TO REGISTER AS A PATIENT

Lavant Road Surgery accepts patients from the area in and around Chichester - from North Marden and Singleton in the North to Hunston in the South.

Our practice area is outlined on the map below.

Reception staff will discuss with you your eligibility to NHS services. If you are eligible, all you need to do is complete a registration form, provide proof of residency (e.g. utility bill or bank statement with address) and photographic ID (e.g. passport or driving licence) for all adults over sixteen in your household.

TEMPORARY RESIDENTS

Patients registered elsewhere in the U.K. who are visiting the area, can be seen as temporary residents by a healthcare professional if necessary.

If an appointment is booked by a temporary patient for travel immunisations there will be an administration charge. The patient must bring records of any other immunisations that they have had previously.

Overseas patients who reside in a country that has a reciprocal agreement with the U.K. who are visiting the area can be seen as temporary residents by a healthcare professional if the need arises.

Overseas patients who reside in a country without a reciprocal agreement can be seen, but a fee will be charged, unless the appointment is medically urgent (immediately necessary).


THE APPOINTMENT SYSTEM

Here at Lavant Road Surgery you are welcome to see any doctor or to see the doctor of your choice when they are next free. However, it isusually helpful to try and see the same doctor about a problem you have previously consulted for as this helps us maintain the continuity of your care.

Please note: Since Government changes in April 2004, patients are no longer registered with a specific GP, but rather with a practice. However, many IT systems within the NHS have not been updated to take account of this and a named GP is still commonly used. If asked for "your GP" please simply quote the practice details

You can make an appointment with a doctor or nurse in person or by telephoning 01243 527264 after 8.00am and before 6.00pm.

We also have an online booking facility. To use this service you will need to register for the online services first.

Please ask a receptionist for your unique password. (You will need to show I.D. (photographic and proof of address) to obtain these details.) The online service will also allow you to request repeat prescriptions.

We try to offer a range of appointments in order to meet demand. It is usually possible to book a few weeks ahead to see a particular doctor for non urgent matters, subject to availability.

There are always ‘On The Day’ appointments to book with a nurse practitioner or a doctor for medically urgent problems that cannot wait until the next routine appointment.

‘Commuter Clinic’ appointments for people who work or are unable to come into surgery during our core hours are available on Monday evenings and on Tuesday and Wednesday early morning.These appointments are not available to book on the day and can only be booked in advance.

Telephone consultation appointments are available during the doctor’s morning and afternoon surgeries. The receptionist will take brief details and a contact telephone number. Usually the doctor will call between 11.00 am and 2.00 pm on the same day for morning appointments and usually after 4.00pm for afternoon appointments, although these times may vary depending on how the surgeries are progressing.

If you wish to leave a message for the doctor to call you outside of these telephone consultations, we will take some brief details as to the reason for the call and a contact number. All calls will be dealt with in clinical priority so we cannot guarantee when the doctor will call you back, It may be up to 5 days.

All of our routine appointments with doctors and nurses are 10 minutes. Please note that one appointment is for the one person only, and ideally for one problem. If you have several issues to discuss please prioritise, you may be asked to make another appointment if more time is needed.

Urgent problems will be seen the same day by the duty doctor (this may mean a 5 minute appointment, depending on demand.) In any event you should be able to see a doctor within two working days.

Doctors or nurses will advise as to check up intervals for those with chronic diseases. Letters will be sent to patients to invite them to make appointments in the relevant clinics.

If you are unable to attend for your appointment, we would appreciate if you would let the reception know as soon as possible.

Please note:

·  Monday session times can vary due to practice meetings.

·  Once a month the practice is closed for a staff training afternoon.

·  Appointment session times may vary during holiday and sickness periods.

THE TELEPHONE

We aim to answer promptly, but in the mornings (especially on Monday) the phones are very busy. To get better service, please avoid telephoning for routine matters such as general advice or test results until after 11.30am.

The telephone lines are open between 8.00am and 6.00pm.

Please be aware that we only accept urgent calls after 6.00pm.

Unless it is an emergency the doctor will not be interrupted during surgery hours.

HOME VISITS

A doctor will visit patients at home if they are unable to attend the surgery for a valid reason e.g. elderly and housebound. Please request a home visit before 10.30 am if possible, giving the receptionist the patient’s name, address, phone number and brief reason for the visit. This makes it possible for the doctor to assess the necessity and urgency of your case and plan their rounds. The doctor may well phone back to discuss the visit.

Unless a patient is elderly or housebound it is more appropriate to see the doctor at the surgery; you will usually be seen more quickly. It is often possible to carry out tests and investigations at the surgery which cannot be done at home.

It does no harm to bring a child with a temperature to the surgery, we have facilities to separate infectious patients from others waiting to be seen.

DISABLED PERSONS

All facilities are available on the ground floor and have wheelchair access.

A wheelchair is available for any patients who have difficulty walking. Please ask at reception.

There are especially designated disabled parking spaces available in our car park.

An induction loop is available at reception to help hard of hearing patients.

CARERS

If you are a carer (a person who provides regular unpaid help) for family members, friends or neighbours who are sick or disabled, then the surgery has a service to support you. For more information, please inquire at reception.

GENERAL INFORMATION

ONLINE ACCESS

In co-operation with ‘SystmOne’, our computer supplier, we are using a service that gives patients internet access to the practice via our website www.lavantsurgery.co.uk

You can:

·  Book, cancel or check your appointments on the web

·  Request repeat prescriptions

Once registered with the practice, you must request to use the Online Services. To ensure that only genuine patients apply for this service, you must come into the surgery together with proof of ID (preferably photographic ID and proof of address) and personally obtain an Online Services registration letter. This letter will provide you with secure information that will allow you to create an online account.

REPEAT PRESCRIPTIONS

Repeat prescription information and a request form is printed on the right hand side of computer generated prescriptions. A repeat prescription can be requested in several ways, either:

o  By dropping the request into the Prescription Request Box in reception

o  By post enclosing an S.A.E

o  By Fax to 01243 530607

o  By e-mail:

o  Online via our website - please see above and ask at reception to register for Online Services.

Two working days notice is required for repeat medication requests so that they can be checked and signed by the doctor. We do not take requests for repeat medication over the phone because experience shows that this can lead to mistakes, something we would all rather avoid. If you wish prescriptions to be collected by a particular pharmacy, then please indicate this when requesting items.

Repeat medications are reviewed at intervals by the doctor. These dates are indicated on the bottom right hand side of the prescription form. You will be reminded when appropriate.

WE WOULD BE GRATEFUL IF YOU COULD HELP TO CUT THE AMOUNT OF MONEY SPENT BY THE NHS ON UNUSED DRUGS BY ONLY ORDERING THE DRUGS THAT YOU ACTUALLY NEED.

Car Parking

Car parking facilities are available at the surgery with designated parking spaces including marked disabled spaces. Please park in the marked bays and please do not block the exit.

Parking is at your own risk. The surgery does not accept any responsibility for any damage that may occur.

Dogs

All dogs, with the exception of assistance dogs, should be left outside the surgery. There are hooks available at the side of the building for their leads to be tied.

Self Check In

There is a self check in touch screen available in reception for patients to mark in that they have arrived. The self check in is simple to use but if you prefer you can check in through the receptionist.

Changes in personal details

Please inform the surgery of any changes in your personal details e.g. address, name or telephone numbers (mobile and landline) as soon as possible so that we can ensure that your records are kept up-to date.

Patient Participation Group

We have a very active P.P.G at the surgery which is open to patients of all ages. The group meets quarterly at the surgery. There is a form included with the registration documents that you can return to the surgery if you are interested in joining the group. New members are always welcome. One of the benefits of joining the group is that it can be a good way to meet new people if you have moved into the area.
