RE: Physicals

The Lehigh Valley Health Network is pleased to provide free sport physicals to all student-athletes in the East Penn School District. In order to provide an easy and effective physical, please adhere to the following directions:

1.  Complete all appropriate sections of the PIAA Comprehensive Initial Pre-Participation Physical Evaluation (PIAA CIPPE Form). In order to be eligible for the physical the following sections must be completed:

Section 1: Personal and Emergency Information

Section 2: Certification of Parent/Guardian

Section 3: Understanding of Risk of Concussion and Traumatic Brain Injury

Section 4: Understanding of Sudden Cardiac Arrest Symptoms and Warning Signs

Section 5: Health History

2.  Section 6 will be completed by the doctors at the physical exam.

3.  All athletes should bring or wear gym shorts, t-shirt and corrective lenses.

4.  Failure to report for a sports physical places the responsibility to complete the PIAA CIPPE form on the athlete and his/her parent/guardian.

5.  No student-athlete will be eligible to participate in interscholastic athletic competition (try-outs, practice or games) until his/her CIPPE form has been delivered to the EHS Athletic Office.

2017-18 WINTER SPORTS SIGN-UP INFORMATION

  • Practice for all winter sports officially begins on Friday, November 17, 2017
  • Every candidate must submit a PIAA Comprehensive Initial Pre-Participation Evaluation Form to the EHS Athletic Office no later than November 10, 2017. Please insure that Sections 1-5 have been completed by a parent/guardian and Section 6 by a licensed physician.
  • The EPSD is offering a free physical on Wednesday, November 1st in the EHS Cafeteria (schedule below):

DUE TO THE LARGE NUMBER OF ATHLETES, STUDENTS WILL NOT BE PERMITTED TO ENTER THE PHYSICAL LINE UNTIL THEIR ASSIGNED TIME.

MIDDLE SCHOOL TRANSPORTATION: immediately following dismissal special buses will be available to students at both schools. All buses will be specifically marked for EHS Physicals.

PIAA RE-CERTIFICATION FORM: students are required to have one (1) sport physical per school year. If a sports physical has been performed more than six (6) weeks prior to the first official day of practice, a PIAA PHYSICAL RE-CERTIFICATION FORM – Section 7 must be completed by a parent/guardian. In addition, students who participated on a 2017-18 fall Eyer/LMMS/EHS sports team must also have a parent/guardian complete the above mentioned re-certification form. In both circumstances, the re-certification form may not be completed earlier than six (6) weeks prior to November 17, 2017. If any Supplemental Health History questions in Section 7 are checked YES or circled, the student must submit a completed Section 8: Certification by a Licensed Physician of Medicine or Osteopathic Medicine to the Emmaus Athletic Office no later than November 10, 2017 in order to be eligible to participate in practice.

PLEASE NOTE: November 1, 2017 is the only date that winter sports physical examinations will be offered at Emmaus High School. If a student does not receive a physical examination on November 1, 2017 at EHS, the student must schedule a physical with a private physician. Any physical examination not performed on November 1, 2017 at EHS must be submitted directly to the EHS Athletic Office no later than 2pm prior on Friday, November 10, 2017. Athletic Trainers and coaches are not permitted to collect physical forms on the first day of practice.
