THE PLAN OF OPERATION IS INTENTIONALLY LEFT AS A MICROSOFT WORD DOCUMENT SO THAT POTENTIAL CONTRACTORS MAY FULLY PROVIDE PERTINENT FACTS REGARDING INFORMATION REQUESTED. In completing the information questions below feel free to expand the Word document into additional pages. If preferred, attachments to this form are acceptable, provided the additional information is identified by question number.

PLAN OF OPERATION:

Give a description in detail of the plan of operation:

1. Personnel:

(A) List the number of full time and part time employees, position titles, and average numbers of hours per position for each facility required by this RFP or proposed by the Contractor.

(B) List name (if known), experience and qualifications of your proposed manager(s).

(C) Provide your organization’s training plan for employees relative to the jobs they perform and clientele they serve.

2. List those items and services you propose to offer for sale/rent at each facility required by this RFP or proposed by the Contractor.

3. Below, list all other item(s) or service(s) you will offer.

4. Describe how you will market the facilities and the state of Ohio. Include your marketing, advertising, and sales promotion plans including staffing and budget specifically for each of these activities.

5. Explain your organization's experience in and plan for ensuring quality customer service, and include examples. If you wish to include customer service materials or manuals, please include these as exhibits at the end of the bid package submittal.

6. Explain your organization's experience in and plan for ensuring the cleanliness of the facilities, and include examples. If you wish to include cleaning manuals or other written materials, please include these as exhibits at the end of the bid package submittal.

7. Explain your organization's experience in and plan for ensuring quality maintenance of the facilities, and include examples. If you wish to include maintenance materials or manuals, please include these as exhibits at the end of the bid package submittal.

8. Describe your organization's plans to hire personnel from Ohio and to use Ohio sources for supplies and materials.

9. At this time, do you plan to sublet or assign any portion or the entire sublease and contract to another party? If yes, explain.

10. * List below your proposed inventory investment relative to equipment, furnishings and other operating equipment. List should also reflect amounts of investment.

11. * For the operation of the facility, how much working capital, over and above the initial inventory investment are you prepared to provide?

12. * For the operation of the facility, approximately what portion of the aggregate of initial inventory investment and working capital do you plan to finance?

13. Provide a pro forma statement for the first two full years of operation projecting your expectations of categorical revenue, categorical expenditures and profits (before taxes).

14. You may, if you desire, provide a narrative that would include additional information you feel might assist the Department in evaluating your plan of operation and pro forma.
