activities associated with participation in the International Fast Pitch Friendship Series will be by team bus/coach, there may be times that it is necessary for team members to travel by
private vehicle driven by team officials. Softball Queensland Inc seeks your permission to transport your
child by private vehicle where and when necessary. Please complete the form below and return it as
