The membership runs from January to December each year. Membership cards will be mailed upon receipt of this application and dues or will be issued at a rodeo. If any of the above information is found to be false, your membership is automatically terminated and your dues, points, and point standings are forfeited. The burden of proof lies with the member in case of question relative to the above information. A photostat copy of your birth certificate must be enclosed with this application.

We, the undersigned parents and legal guardians of ______, do fully agree and consent to the participation of our child ______, in any and all youth rodeos and practice days for the rodeo approved by The Southern Tennessee All-Star Rodeo Association and in consideration for permitting our child ______to participate as a contestant in such rodeos and practice days for the rodeo and or other events pertaining to the rodeo, we hereby consent and fully acknowledge the following: STAR members acknowledge that rodeos and practice days for the rodeo are dangerous activities and that participation in a rodeo or practice days for the rodeo as a competitor, an independent contractor, or volunteer, exposes the participant to substantial and serious risk of property damage, personal injury, or death. STAR members acknowledge that participation in STAR sanctioned rodeos or practice days for the rodeo will involve such a hazard. Being fully aware that participation in STAR sanctioned rodeos and practice days for the rodeo will expose said member to substantial and serious risk of property damage and/or personal injury or death, said member releases STAR, sponsors, and STAR sanctioned rodeo production entity, their affiliated, related or subsidiary companies, and the officers, directors, employees, and agents of each entity or organization from liability for any and all property damage, personal injuries, or other unknown, foreseen and unforeseen, future or contingent. STAR members shall not now or any time in the future, directly or indirectly, commence or prosecute any action, suit, or other proceeding against the before mentioned arising out of or related to the actions, causes of action, claims and demands hereby waived, released or discharged by STAR member. This provision shall be binding upon each STAR member, his/her spouse, legal representative, heirs, successors, and assigns.

By allowing our child ______to participate, I am aware that rodeo is a vigorous sport involving severe cardiovascular stress and violent physical contact. I understand that it involves certain risks, including but not limited to, death, serious neck and spinal injuries, resulting in complete or partial paralysis, brain damage, and serious injury to virtually all bones, joints, muscles, and internal organs, and that equipment for my protection may be inadequate to prevent serious injury. In addition, I understand that participation in the STAR Rodeo involves activities incidental thereto, including, but not limited to, travel to and from the site of the activity, participation at sites that may be remote from available medical assistance, and the possible reckless conduct of other participants, I am voluntarily, and for my child’s and my own interests, participating in this activity with knowledge of the danger involved and herby agree to accept any and all risks of property damage, personal injury, or death.
