7-1-1 Alpha Test Feedback Form

*** Below is a sample of an alpha test feedback form. The names of the structural elements reflect the actual course for which this form was designed. You will need to adapt the structural elements to reflect the design of your course / resource. Also, you may want to add additional questions to suit your course / resource needs.

Each of you who worked on the build of Course / Resource xxx received an ALPHA version and this comment sheet. Comment sheets are to be turned into xxx by <date>. Your comments will be reviewed and the necessary revisions made. As soon as possible, a beta version will be distributed to selected schools via IP satellite. A formal course / resource evaluation will be completed by those taking the beta version of the course. If it is determined that revisions are needed, we will address it at that time.
