Pressure Ulcer Prevention ROP

·  Pressure ulcers have a significant impact on client quality of life resulting in pain, slower recovery and increased risk of infection.

·  Effective pressure ulcer prevention strategies can reduce the incidence of pressure ulcers and are an indication of higher quality care and services.

Tip:

o  Team members, clients, families and caregivers are provided with education regarding the cause of pressure ulcers and the interventions being provided to prevent them.

I need to know:

·  Health PEI Pressure Ulcer Prevention Policy is located on the Policy Document Management System (PDMS).

·  Health PEI uses the Braden Assessment to provide early identification of individuals at risk of developing a pressure ulcer. A risk score is assigned based on the assessment.

·  A Braden Power Plan is ordered based on the patient’s risk score. The Power Plan identifies which pressure ulcer prevention interventions to initiated for the patient’s care.

·  Patients are assessed within 24 hours of admission or 24 hours post operatively.

·  Patients are re-assessed every 7 days and with any patient status change.

·  Quality Teams audit compliance with Braden Assessment and associated interventions.

·  Hospital acquired pressure ulcers can be tracked through the Patient Safety Management System (PSMS). These are captured in the General Event Type “Skin/Tissue”

·  The ROP Handbook can be found on the Health PEI Staff Resource Centre.

Required Organizational Practices (ROPs) are identified by Accreditation Canada as evidence based-practices, which reduce harm to patients. They are mandatory practices matched to specific Accreditation Canada Standards that organizations must have in place. There are 33 ROPs identified within Health PEI for the September 2017 survey visit.
