Alliance "Find a Doctor" Sweepstakes Official Rules
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS GIVEAWAY.
ELIGIBILITY:
To enter the Alliance "Find a Doctor" Giveaway (the "Giveaway"), you must be (a) a citizen of the United States or a lawful permanent resident of the United States; (b) 18years of age or olderat the time of entering; and (c) a registered user of the Alliance "Find a Doctor" website located at Only employees and their dependents of existing Alliance members who are on a plan using The Alliance network are eligible to register with the "Find a Doctor" website. The Giveaway is void where otherwise restricted or prohibited by law.
How To ENTER AND PARTICIPATE:
All eligible entrants who log in to theFind a Doctor website between November 1, 2016 and December 31, 2016 shall be automatically entered to win the Giveaway.
DRAWING & NOTIFICATION:
One (1) grand prize winner and five (5) winners will be selected by The Alliance on or around January 31, 2017. Winners will be sent an email message using the email address associated with their Find a Doctor account information. Selected winners will be required to respond to the prize notification within fifteen (15) days verifying their contact information. If a selected winner does not respond to the notification email within fifteen (15) days from the first notification attempt, then such entrant will be disqualified and an alternate winner will be selected. Prizes will be mailed within thirty 30 days.
The odds of winning depend on the number and quality of eligible entries received, and the winners shall be determined by The Alliance, in its sole and absolute discretion and The Alliance's interpretation of these Official Rules. All decisions are final and binding in all respects.
PRIZES:
One (1) grand prize winner and five (5) winners will be randomly selected from all eligible entrants. The grand prize winner will win one (1) $250 Visa gift card. Five (5) winners will receive one (1) $50 Visa gift card.
Winning entries and prizes are not transferable. No prize substitutions are allowed except by the Alliance who reserves the right to substitute prizes with prizes of equal or greater value. Other terms and conditions and restrictions from Visa may apply.
MISCELLANEOUS:
This Giveaway is subject to all applicable federal, state and local laws. By participating, entrants agree to be bound by these Official Rules and the decisions of the Alliance and waive any right to claim ambiguity in the Giveaway or these Official Rules.
The Alliance shall not have any liability for (i) any technical failures of any kind, including but not limited to malfunctions, interruptions or disconnections in internet connections, network hardware or software, email addresses or Find a Doctor account access; and (ii) technical or human error which may occur in the administration of the Giveaway. The Alliance may prohibit an entrant from participating in the Giveaway or winning a prize if, in its sole discretion, the Alliance determines that said participant is attempting to undermine the legitimate operation of the Giveaway by cheating, hacking, deception or other unfair playing practices (including the use of automated quick entry programs or methods) or intending to annoy, abuse, threaten or harass The Alliance or any other participants.
If for any reason this Giveaway is not able to be conducted as planned, including, but not limited to, by reason of infection by computer virus, bugs, tampering, unauthorized intervention, fraud or any other causes beyond the reasonable control of The Alliance which corrupt or affect the administration, security, fairness, integrity or proper conduct of the promotion, then The Alliance reserves the right at its sole discretion to cancel, terminate, modify or suspend the Giveaway.
The Alliance reserves the right to revise, amend, suspend or end the Giveaway, or portions of the Giveaway, if any portion of the Giveaway is deemed to violate any local, state or federal regulations without liability to any participants.
