VACANCY NOTICE

Position: Investigative Agent

Position reports to:Director of Program Operations and Superintendent

Minimum Requirements:Must meet the certification requirements of the Ohio Department of Developmental Disabilities pursuant to Administrative Rule 5123:2-5-07.

Work Day: 8:00 am – 4:30 pm

Work Year: 260 days per calendar year

Responsibilities:The Investigative Agent shall be responsible for reviewing all unusual incidents for trends and patterns, and reports these trends and patterns to the Director of Program Operations and the Superintendent. The Investigative Agent shall investigate any major unusual incidents including cases of suspected abuse, neglect, missing persons, misappropriation of funds, deaths, cases involving law enforcement, attempted suicides, fires, natural disasters, mechanical failures resulting in the relocation of the individual(s), medical emergencies, unplanned hospital admissions, any injury of unknown or suspicious origin, the use of behavior support methods including restraint or time out, any violation of rights as enumerated in Section 5133.62 of the ORC, or a series of unusual incidents. The Investigative Agent shall comply with Administrative Rules 5123:2-5-07, 5132:2-17-02, and ORC Sections 5126.31 and 5126.313.

Position Description:

Upon receipt of a major unusual incident, the Investigative Agent shall review the incident to ensure that the provider has taken all reasonable measures necessary to protect the health and safety of the individual and determine if any additional action(s) must be taken.

The Investigative Agent shall immediately commence an investigation using the Investigative Protocol if a major unusual incident involves abuse, neglect, misappropriation of funds, any injury of unknown or suspicious origin, suspicion of accidental death, an individual who cannot be located, or any other major unusual incident the County Board determines should be investigated.

The Investigative Agent shall submit a report to the Department of Developmental Disabilities within 30 working days. The report shall be submitted through the online system established by the DODD.

The Investigative Agent shall provide a written summary of the investigation findings to the individual, advocate, legal guardian, parent, provider, service coordinator, Director or Program Operations and Superintendent.

The Investigative Agent shall make recommendations to close a case based on the findings of the investigation.

The Investigative Agent shall review and analyze, on a weekly basis, unusual incidents and major unusual incidents and identify patterns and trends specific to an individual.

The Investigative Agent shall prepare a quarterly report using information contained on the online system established by the DODD for each provider in the County. The report shall identify the numbers and types of incidents that have occurred.

The Investigative Agent shall communicate with the Service Coordinators, department administrators, Director of Program Operations and Superintendent concerning investigations that are conducted.

The Investigative Agent shall conduct an internal review of all Unusual Incidents Reports to ensure that appropriate action has been taken to protect the health and safety of individuals and patterns and trends have been identified and reviewed.

The Investigative Agent shall review Unusual Incident Reports weekly to determine whether a series of unusual incidents have occurred that could indicate that a major unusual incident has occurred.

The Investigative Agent shall review all Unusual Incident logs maintained by providers, on a quarterly basis, to ensure that patterns and trends have been identified and ensure that corrective action has been taken.

The Investigative Agent shall participate in Ohio Department of Developmental Disabilities training and other in-service programs as assigned by the Superintendent and Director of Program Operations.
