5. Inspection has demonstrated the machine, cutter, and pilot bit are in satisfactory condition.

Conditions

1. If the permit to work process is in use, this permit is supplementary to the Permit to Work and may not be used alone. Refer to Practice 000.653.1302, Permit to Work.

2. This permit must be presented when performing a hot tap.

3. This permit is valid only for the job described on it.

4. In the event of damage or striking unexpected obstacles (including service pipes and cables), stop work immediately and report to the Hot Tap Permit Issuer who issued this permit.
