COURSE DESCRIPTION This course provides the student with clinical experiences necessary for the management of adult health, including wellness promotion, and illness prevention and treatment in post-pubescent adults from diverse backgrounds. Emphasis is on the application of theoretical principles, assessment skills, critical thinking and evidence based practice to formulate differential diagnoses, clinical impressions, diagnoses, and treatment and evaluation plans for adults with multi-system complex diseases in both acute care and out-patient settings.

COURSE OBJECTIVES Upon completion of this course, the student will be able to:

1.  Apply the concepts of wellness promotion, illness prevention and treatment, in the advanced nursing management of adults with selected multi-system complex health problems in acute care and out-patient settings.

2.  Integrate current research findings, evidence based practice guidelines, and standards of care into the management of multi-system complex adult health problems.

3.  Assess adult clients presenting with multi-system complex health care problems comprehensively and accurately to diagnose and develop differential diagnoses.

4.  Implement individualized comprehensive management plans for adults with multi-system complex health problems.

5.  Evaluate the effectiveness of management plans in achieving optimal client outcomes for selected multi-system complex adult health problems.

6.  Collaborate with the interdisciplinary health care team in facilitating optimal health care outcomes for adults with selected multi-system complex health problems.

7.  Integrate cultural, legal and ethical principles to guide decision-making in the advanced nursing practice role.

8.  Demonstrate professional verbal and written communication skills appropriate for the advanced nursing practice role.

CLINICAL SCHEDULE

Variable hours: A total of 96 clinical hours are required and must be accumulated by August 5th, 2011 of which one (1) hour will consist of a mandatory CON clinical orientation and two (2) hours will consist of mandatory online clinical seminars (total 3 hours). Each student can use a maximum of 4 clinical hours for clinical site/course orientation. (Refer to “Clinical Hours Log” on website) Each student is responsible for notifying their faculty member of their clinical schedule. Any changes in the clinical schedule require immediate notification of the student’s faculty member by phone and e-mail communication (see below).

Students are responsible to notify the course faculty via cellular phone call, at the time that any extraordinary scheduling circumstances may occur, such as student or preceptor illness on a planned clinical date/time. Leaving messages on faculty office phones is not acceptable. After notification of faculty member, students are to follow up by submitting the change to the faculty in writing (via e-mail).

Sakai is the course management system that you will use for this course. Sakai is accessed by using your Gatorlink account name and password at http://lss.at.ufl.edu. It is important that you regularly check your Gatorlink account email for College and University wide information and the course Sakai site for announcements and notifications.

Course websites are generally made available on the Friday before the first day of classes.

ATTENDANCE

Students are expected to be present for all scheduled clinical practice experiences and seminars. Students who have extraordinary circumstances preventing attendance should explain these circumstances to the course instructor prior to the scheduled clinical practice experience or seminar. Instructors will make an effort to accommodate reasonable requests. A grade penalty may be assigned for unexcused seminar or clinical absences.

Unexcused absences will result in a zero score and unsatisfactory performance for scheduled activity. Absences associated with extenuating circumstances may be assigned an (I) in the course at the Instructor’s discretion and will need to be made-up before the course is successfully completed.

Graduate students are required to submit a written calendar of planned clinical practice dates and times to the course faculty member prior to beginning the clinical rotation. Any changes to the calendar (dates and times) must be submitted in writing to the course faculty member before the change is planned to occur. Clinical hours accrued without prior knowledge of the faculty member will not be counted toward the total number of clinical hours required for the course.

Assignments are to be submitted by scheduled submission dates.

ACCOMMODATIONS DUE TO DISABILITY

Each semester, students are responsible for requesting a memorandum from the Office for Students with Disabilities to notify faculty of their requested individual accommodations. This should be done at the start of the semester.

STUDENT HANDBOOK

Students are to refer to the College of Nursing Student Handbook for information about College of Nursing student policies, honor code, and professional behavior. Of particular importance for this course are the sections on appearance in clinical practice areas, personal liability insurance, and student safety.

TEACHING METHODS

Supervision of student clinical practice experiences and seminar/discussion forums/assignments.

LEARNING ACTIVITIES

Discussion forum interactive activities, case presentations, performance of physical exam, dictation and written documentation activities, analysis of research articles & supervised clinical practice.

EVALUATION METHODS/COURSE GRADE CALCULATION OR CLINICAL EVALUATION

Clinical experience will be evaluated through faculty observation, verbal communication with the student, written work, and agency staff reports using a College of Nursing Clinical Evaluation Form (at midterm -after 50% of the total required clinical hours have been completed and at the end of the course, on or before April 20, 2011). Faculty reserve the right to alter clinical experiences, including removal from client care areas, of any student to maintain patient safety and to provide instructional experiences to support student learning.

Evaluation will be based on achievement of course and program objectives using a College of Nursing Clinical Evaluation Form. All areas are to be rated. A rating of Satisfactory represents satisfactory performance and a rating of Unsatisfactory represents unsatisfactory performance. The student must achieve a rating of Satisfactory in each area (refer to list below) by completion of the semester in order to achieve a passing grade for the course. A rating of less than satisfactory in any of the areas at semester end will constitute an Unsatisfactory course grade.

Students must complete ALL of the following criteria to successfully pass the course:

1.  Maintain patient safety in the clinical setting.

2.  Satisfactory demonstration of advanced practice professional accountability to include:

a.  Compliance with attendance and appearance guidelines as described in this syllabus/ addendum, and the student handbook.

b.  Complete and submit written assignments within established guidelines, time frames and expectations as described in this syllabus/addendum, and earn satisfactory grade.

3.  Satisfactory performance in the clinical setting as indicated using the NGR 6244L Clinical Evaluation tool (refer to website).

4.  Complete the scheduled computerized diagnostic practice certification exam on campus (students will sign up for a testing date/time).

The faculty member will hold evaluation conferences with the student and clinical preceptor at each site visit. The faculty member will document or summarize each conference on the Clinical Evaluation Form (Graduate Clinical Visit Evaluation Tool; refer to website) or Incidental Advisement Record. Mid-rotation evaluation conferences will be made available to each student. Final evaluation conferences with the faculty member are mandatory and will be held during the last week of each clinical rotation. The student will complete a self-evaluation of his/her clinical performance by using the College of Nursing Clinical Evaluation Form, and will bring a copy for review during the final evaluation meeting. A student may request additional conferences at any time by contacting the clinical faculty.

Mid-term and final grades for the course are assigned by the clinical faculty member supervising the student.

Students enrolled in advanced practice courses with a clinical component will use Clinical Experience Form F to document clinical experiences including hours, practice location and preceptor for their personal records. Students also assess their learning experience using Clinical Site Assessment Form G. Completed Form G is collected by the clinical faculty member and is submitted to the Coordinator of Clinical Resources at the College. At the end of the clinical experience the faculty member completes a student evaluation using the College of Nursing Clinical Evaluation Form.

Adult Nurse Practitioner 2 Addendum

MINIMAL REQUIREMENTS FOR APPEARANCE IN CLINICAL PRACTICE AREAS
1. / Graduate students are identified with the Health Science Center ID badge in clinical settings.
2. / Graduate students wear clean, pressed, white lab coats over professional attire in clinical settings.
3. / Overall appearance conveys a professional image. This includes as a minimum:
·  Minimal jewelry / ·  No perfumes/scented lotions/etc.
·  Minimal makeup / ·  No artificial fingernails or nail polish
·  Hair neatly secured away from face / ·  Neat, short fingernails (not visible from the palmar surface of the hand)
4. / Personal hygiene and grooming are of a standard that ensures the safety and comfort of clients.
5. / Students arrive in clinical areas with all the required equipment (e.g., stethoscope) necessary for client care.
6. / Activated cell phones and/or pagers are not allowed in either the clinical or classroom setting.

STUDENT SAFETY STATEMENT

Clinical experiences by their nature involve students in a variety of settings, locations and communities, as well as with a variety of patients/clients. Students are expected to exercise good judgment and reasonable caution in insuring their own safety during clinical experiences (e.g., locks car doors, travel with classmates when possible, be aware of security services). Patient care areas may have the potential for exposure to hazardous substances such as radioactive materials. Students who require protection beyond those of all staff are to notify faculty prior to any clinical assignments. If any time students believe the clinical setting is unsafe, students should take appropriate steps to protect themselves and their patients, including leaving the setting if necessary. Contact the course instructor or any college administrator immediately so that appropriate arrangements can be made.

CLINICAL DOCUMENTATION EXPECTATIONS

Each student is responsible to submit completed copies of the following material at the following scheduled submission dates (5/20/11 & 7/15/11):

1.  Record all patient experiences (in compliance with HIPAA regulations) for each clinical date (refer to website for Patient Summary Log Form).

2.  Accurately record and have signed by the preceptor, all completed clinical hours for each clinical day (refer to website for Clinical Hours Log Form).

·  Clinical Hours Log signed by the preceptor (including all data from the start of the semester to the day before submission of the paper; refer to website for forms)

·  Patient Summary Log (including all data from the start of the semester or previous submission date to the day before the form is submitted; refer to website for forms)

·  Forms A and G (refer to website) need to be completed by the end of the semester on or before 7/15/11.

3.  Completion of Clinical Seminar Activities on scheduled due dates: (refer to website).

Online Clinical Case Scenario: Seminar Presentation Guidelines

Each student is responsible to: Postings in Sakai under “Discussions”

1)  Participate in all scheduled on-line seminars (total of 2). Seminar absences associated with extenuating circumstances will result in an “I” in the course and will need to be made-up before the course can be successfully completed.

2)  Unexcused absences will result in a zero score and unsatisfactory performance for scheduled seminar activity. A rating of less than satisfactory in any of the areas at semester end, will constitute a course grade of U.

3)  Seminar participation is graded using the Online Clinical Case Scenario Presentation: Evaluation Criteria as posted on the webpage. Students will accrue a total of 3 clinical hours for seminar participation as assigned (successful completion of all seminars with a satisfactory score is required).

4)  All seminar presentations/responses should be accompanied by at least one relevant specific reference from the current research literature (2006-2011). Textbooks or online text resources will not be accepted as the sole reference for any answer.

o  Responses must be HIPAA compliant.

o  APA format is required for all references.

o  The level of evidence that the reference represents and a brief description of qualifiers that justify the level of evidence assigned must be included.

o  Responses should be clear, concise, and scholarly. Students are expected to integrate discussion consistent with graduate level line of reasoning.

5)  Unsatisfactory score in a seminar activity

o  Students that obtain an unsatisfactory score after completing a seminar activity will have one (1) opportunity to participate in another seminar.

o  An average between original submission score and resubmission score will be obtained to assign a grade to this activity.

o  Students that do not obtain a passing score after an average is obtained, but obtain a score that demonstrates improvement (80 or higher) will receive the minimum passing score allotted for that assignment (80).
