This Permit provides for coordination, identification and marking of CAP buried facilities/utilities and systems locations on and within CAP right-of-way. When completed and approved, authorizes the requesting entity to dig/excavate only in the areas approved by this Permit. If any digging/excavation is required beyond the boundaries of the approved area, then a new Permit is required prior to any digging/excavation.

Requestor must attach drawing(s) to this request showing location(s), depth(s) and route(s) of proposed digging/excavation and boundaries of area for which you are requesting digging/excavation authorization.
Entity requesting digging/excavation authorization must also notify Blue Stake for location and identification of other CAP facilities/utilities.

Box 43020, Phoenix, AZ 85080-3020, Attn: Inspector Supervisor,Steven Golding or hand carried to the CAP, Engineering Services, Inspection - Surveys Division, 23636 North 7th Street, Phoenix, AZ 85024 or faxed to CAP at (623) 869-2249.

Log in request and assign Permit Number, review for requirements. Obtain from Engineering Lands & Records and attach copies of file drawings which involve the area for which permit is being requested. Coordinate any location requirements with the Requestor.

Review comments and/or new attachments, forward document to Inspection - Surveys for field location of CAP buried facilities/utilities.

Review documents and prints attached and assign to Inspector for locating and marking of CAP buried facilities/utilities.
Inspector / All CAP buried facilities/utilities fully located and marked. Drawings revised if location of buried facilities/utilities different than shown on drawings.

Notes to Requesting Entity
If, during digging/excavation, additional utilities are identified or marked utilities cannot be located, excavation activities will cease and Engineering Services will be notified. If a utility or system breach occurs, immediately notify the CAP Control Center at (623) 869-2530.8/18/15 RR
