As from 18th November 2013, a new way of accessing mental health services will open in the North of Lambeth called the Living Well Network Hub. The catchment area for the hub is outlined in the appendix.

Referrals to secondary care will come via the hub, including all referrals previously referred to the Psychosis and MAP (Mood, Anxiety and Personality Disorder) teams. Referrals to Lambeth Early Onset (LEO), OASIS,Talking Therapy and Crisis services will remain the same.

What will be different?

There will be a wider remit of access to clinical and mental health services.The referral criteria are:

People identified by their GPs and/or other agencies as experiencing significant depression or anxiety or other mental health problems in the context of significant social stressors, who may a) be receiving treatment for their mental health problem in primary care b) require additional assessment and support to alleviate distress or prevent deterioration into crisis and c) who do not met the criteria for specialist mental health services.

There will be a new integrated clinical and social care triage service. A multi-disciplinary team of senior practitioners who were previously working in the community mental health teams (from nursing, social work and occupational therapy backgrounds) will work with nurses from primary care (the PASS Team) and the voluntary sector (Community Options) to identify needs quickly and triage to appropriate support.

A 12 week re-ablement offer will be available. This means that people will be offered up to 12 weeks of intensive modular support. Modular support means tailored support from agencies involved in the Network. This includes psychiatry input, specialist voluntary sector support around someone’s daily living skills, employment, benefits etc.

There will be access to peer support(people who have lived experience of using mental health services).

There will be a wider menu of options to support people, including personal health budgets, access to services to support people in their community such as Community Connections, and the Living Well Partnership.

The network will begin to work in GP surgeries

Referral process

At the moment, referrals can be made via a GP, or another mental health professional. This is a prototype, and we are constantly wanting to improve our processes and user experience. We will be piloting a new introduction form soon, but for now, referrals can be made on existing referral forms, or via letter, fax or telephone. We would encourage you to pass on views about how the new referral process is working for you and for the people you are referring.

Why is there a new access point?

The Collaborative (a group of GPs, providers across statutory and voluntary services, commissioners and service users and carers) have engaged with over 1700 people since June 2010 to design a system of care that better meets people’s needs. The ‘Big’ Three outcomes we wish to achieve are for people to:

1) recover and stay well,

2) make their own choices and

3) participate on an equal footing in daily life.

Discussions identified that:

-It is difficult for some people to access clinical and social support in the current system quickly enough to prevent crises. Sometimes if people had been supported earlier, referral to secondary care could have been avoided.

-There are lots of resources and services in the community but confusion as to identifying what they are and how to access them

-We need to support people to be more connected to their communities so that they have robust support networks to help them when needed.

As a Collaborative we are therefore ‘turning the system on its head’ and are putting more support in primary care to help people stay at home.
