Brief Summary of What You Need To Do BEFORE You Can Commence provision of the Service

Return the following completed documentation

1- Signed Contract (one copy only- you keep one)

2- Declaration of Competence (CPPE)

General Information

There are several requirements prior to commencing provision of this service.

  1. Signed NHS Contract for Minor Ailments Scheme
  2. Read and Sign the contract
  3. Return one signed copy to the CCG using the contact details above.
  4. NBif the pharmacy is part of a pharmacy group, the contract document may have been signed by a representative of the group at Head Office. It is your responsibility to confirm this prior to commencing the MAS. If signed centrally, only the pharmacist’s DoC is necessary to submit. (applicable for Boots, Lloyds, & Rowlands Pharmacy groups)
  5. The Contract holds the details of the scheme, so keep a copy to hand (electronic copy is availablefrom LPC website if preferred)
  1. Declaration of Competence (DoC) on CPPE
  2. Goto the website
  3. Login using your CPPE details (you should know these)
  4. Select my services & declaration of competence (this page will then display useful information which will answer your questions re DoC)
  5. Select hyperlink “access the DoC for a specific service”
  6. Select “minor ailments”
  7. View, complete, download/save and print your DoC document (please note Level 2 PGD section does not apply currently for this scheme)
  8. Send a signed copy to the CCG using the contact details below as soon as possible
  1. PharmOutcomes Validation

Once these two requested documents are received at the CCG, we will addthe new service to your PharmOutcomes Services profile. You then need to log in to PharmOutcomes to activate the service. It requires an initial setup, which will take just a few moments.

  • Visit
  • Enter login details
  • Select “Services” along the top bar
  • Select “Minor Ailments Stage 1 Registration”
  • Follow the details which will link with the CPPE website to obtain confirmation of the DoC being downloaded (but not signed- this is why we need the hardcopy). Once this is done you will receive a confirmation on screen, and then be ready to use the “Stage 2 Consultation” option which is where you capture details of the supply each time you provide the service.

  • Scroll down to Service 11 (if not available- try again later)
  • Download a copy of the Contract, which contains details of the scheme within various headings.

NB there are many parts of the NHS Standard contract which do not apply to this service, but remain in the document layout.

If you are returning the completed and signed Declaration of Competence (DoC) form separately from the signed contract, then the signed DoC may be sent in the following ways:

  • By post to the address above
  • A scanned signed copy emailed to (please provide a good quality .jpg file)

We do not wish to receive Faxes, as the reproduction quality is often poor.

For queries which cannot be answered by reference to the Contract including details of the scheme, or the various appendices provided, please telephone the Medicines Management team on 0300 421 1747, clearly stating it is a Minor Ailment Scheme query. You will be directed to the most appropriate person available at that time.
