MEMBERS PRESENT: Mona Rainey, Carlton Starke, Elsie Gladding, (new members) Jackie Hinman, Sofia Howe and Kathy Boyd.

CALL TO ORDER: The meeting was called to order by Mona Rainey, Co-Chairperson, at 12:07 p.m. A quorum of the LHRC Board was present to conduct official business.

INTRODUCTION OF NEW BOARD MEMBERS: The LHRC Board members introduced themselves. Three new members were also present to introduce themselves to the Affiliate members.

INTRODUCTIONS: Beverly Garnes, Regional Advocate also introduced herself to those present and welcomed the new LHRC Board Members.

APPROVAL OF DRAFT MINUTES: The LHRC Chairman requested the April 22 2015, LHRC Meeting Minutes be approved as written. After a motion by Carlton Starke, the motion was seconded by Jackie Hinman, to approve the minutes as submitted. All members of the LHRC were in favor and the meeting minutes were approved as submitted.

PUBLIC COMMENT: There was no public comment presented at today’s meeting.

OLD BUSINESS:

·  Mona Rainey, Co-Chair again mentioned the concern by this LHRC as it relates to Affiliates completing their Quarterly Reports on time and in their entity as well as attending meetings.

·  Affiliate members stated they were not getting all emails relating to LHRC business. Ms. Rainey reminded everyone to update their email addresses on the sign-up sheet and to keep them current.

NEW BUSINESS:

·  As a follow-up to the Financial Support Presentation by Donald Burge, Executive Director, Southside Community Services, at last quarter’s meeting, Dee Holland-Brock, Affiliate/ Southside Community Services requested to re-visit the process and presented to the LHRC a report from Gayle Grainer, Finance Department/ SSCS, of those Affiliates who have not made a “contribution” so far this year and suggested it was in the best interest of the Affiliate members to formulate a Committee to address this concern and to have a reporting entity on expenses of the Affiliates.

·  Tess Shipe, Affiliate Member, Behavioral Health & Wellness Center, reported as a member of another LHRC, noted the process involved and tracking purposes of this process. Ms. Shipe agreed to support the Affiliate Members in any manner possible.

·  Dee Holland-Brock asked in order to move forward with this concern, a recommendation from the floor of Affiliates to nominate someone or a Committee to oversee this process. All Affiliates were in favor of this process by a show of hands from the floor. Dee Holland-Brock nominated Teri Bistarkey, Community Living, Inc. and she accepted. Tess Shipe also agreed to support Ms. Bistarkey.

·  Members voted on new LHRC members. A motion was made by Kathy Boyd for Mona Rainey to become Chairperson. This was seconded by Carlton Starke. All were in favor. A motion was made by Mona Rainey for Carlton Starke to be Co-Chair. This was seconded by Kathy Boyd. All members were in favor. Jackie Hinman accepted the position of Secretary. This was motioned by Carlton Starke. It was seconded by Sofia Howe. All were in favor. As it relates to the Subcommittee to handle LHRC business, Kathy Boyd, Mona Rainey and Sofia Howe all was nominated and accepted to serve on this committee. It was motioned by Carlton Starke and seconded by Sofia Howe. All were in favor.

REGIONAL ADVOCATE’S REPORT: Ms. Garnes reported on the CHRIS reports for Abuse and Complaints for Quarterly Reports for Affiliate members and provided a handout for further explanation. She requested Affiliates to pay close attention to the reports and when submitting their Quarterly Reports. The Affiliates should submit the New “CHRIS Abuse Report-AB-07” instead of Old “CHRIS Abuse Report-AB-01.” Ms. Garnes stated that if the affiliate does not have any CHRIS Abuse or Complaint data for the reporting period, it is not necessary to attach a blank CHRIS report to their quarterly report. Affiliate members asked if serious incidents/injuries needed to be reported at all the times. Ms. Garnes noted anytime there was a serious incident/injury that required medical attention from a medical professional or seen at the ER or by a physician or is escorted to a hospital for any reason, the incident must be entered into CHRIS as a serious incident. Regular scheduled medical appointments do not need to be entered into CHRIS.

Revised Human Rights Regulations are currently on the Governor’s desk for review.

Mrs. Garnes reminded Affiliates to put their licensing number on Quarterly Reports.

AFFILIATE REQUESTS: Tess Shipe, Director of New Business for Behavioral Health and Wellness Center (BHWC) presented information to the LHRC, requesting affiliation to the committee. Ms. Shipe reported that BHWC was looking to provide mental health skill building in the Chase City/ Clarksville area. A motion was made by Carlton Starke to accept Behavioral Health and Wellness Center as an Affiliate Member. It was seconded by Jackie Hinman. All LHRC members were in favor.

AFFILIATION REPORTS:

A.  Another Beginning, Inc.--Kelly Carr: Currently serving 22 individuals in mental health supports. No reports of abuse or neglect. Serving 6 individuals in IIHS. No reports of abuse or neglect. One new staff member. Licensing was out today for license renewal.

B.  Caring Cove, Inc.: not present

C.  Community Living, Inc.-Teri Bistarky: Served 18 clients. No incidents of abuse or neglect. No changes to program or staff.

D.  Community Interventions, LLC-Edna Phipps: served 12 clients with no incidents of abuse or neglect, no licensing changes. Human rights issues and training are discussed and presented in monthly staff meetings and supervision.

E.  Destini Therapeutic Services-Bernadine Johnson: 44 clients for mental health support and 8 clients for IIHS. No allegations of abuse or neglect last quarter.

F.  Family Alternatives, LLC.-Stacy Hicks-Macklin: No clients at the present time.

G.  Family First-Susan Pontoon: 2 IIHC client. No changes in policy or procedures. Family First has been talking to a perspective new LHRC Board Members. Family First has recommended one person whose is on the current LHRC Board.

H.  Family Sharing: not present

I.  G & H Group Home-Harriet Outlaw: Currently serving 15 individuals with 8 in residential/ 7 in day support. No reports of abuse or neglect. No program changes.

J.  Hope First: No allegations of abuse or neglect. 5 IIHS clients and 19 mental health skill-building clients served.

K.  Life Young Adult Services, LLC-Michael Wright: No reports of abuse or neglect. Serving a total of 24 individuals (18 MHSB/ 6 IIH). Staff trainings are used as efforts to ensure allegations of abuse/ neglect and human rights complaints are captured and reported as required by regulations.

L.  New Journey Family Services-Ruth Wilson: No allegations of abuse/ neglect. Serving no clients at this time. No complaints at this time.

M.  North View-Beverly Hargrove: No changes/ no allegations of abuse or neglect. No clients served during last quarter.

N.  Olmeja-Leslie McInturf: 2 clients served. No reports of abuse/ neglect. 2 new staff. No licensing changes or policy changes.

O.  Palmer Springs Group Home/ Palmer Springs Psychological/ H &R Support Services/ Alberta Day Program-Katrina H.: 2 clients in H & R supports, with no incidents or allegations. 26 clients in Palmer Springs Day Program with no incidents of abuse/ neglect/ allegations. 6 clients in Palmer Springs Group Home with no incidents of abuse or neglect/ no allegations. 5 clients in Hazels Group Home with no incidents of abuse or neglect. 22 clients in Alberta Day Program with no incidents of abuse or neglect/ no allegations.

P.  Pathways Youth Services, LLC-Sharon Chism-Medley: one incident reports in late May involving two runaways. The situation involving staff neglect was reported, investigated, and resolved with proper disciplinary procedures, which occurred in house. Served 4 clients during the quarter. However, as of the meeting date, currently serving 3 residents. No amended policies or changes to program rules.

Q.  Perfect Timing-Chris Davis: Waiting on licensing.

R.  Raven’s Nest-Raven Jackson: Annual license is affective 4/30/15. No allegations of abuse or neglect. Currently serving 3 male residents and three new staff have been hired part time.

S.  Roll’N 2 Success, LLC.: not present

T.  Safe Haven, Inc.-Grace Ilor, Alisa Crawley; Felicia Robertson: Serving 2 consumers. No allegations of abuse or neglect/ no program changes.

U.  Safe Harbor Group Home, LLC-Willie Moseley: No clients. No changes.

V.  Sentara Halifax Regional Health Services-Catherine Wright: 108 patients served in outpatient and group therapies. No reports of abuse or neglect. No changes in policy or procedures.

W.  South Central Counseling Group: not present.

X.  Southside Community Services Board-Dee Holland-Brock: Served 2085 clients last quarter (51 EI, 1625 Mental Health, 229 ID, 182 SA; 359Emergency Services & VICAP. 30 incidents of Peer to Peer. 11 Physical/ 9 verbal/ 15 peer to peer. 15 closed and 15 open cases. 8 deaths reported (4) natural, (1) suicide; (3) undetermined. No complaints at the Executive Director Level. Staff changes at the Administrative Level: Jane Rice, Financial Director resigned. Beth Englehorn is now Acting Director of Behavioral Health.

Y.  The Center for Child and Family Services, Inc.: not present

Z.  Turning Point Youth and Family Services, LLC.-Angela Edmond Served 27 consumers through IIHS. No allegations of abuse or neglect. No changes in program. No policy changes.

AA.  United Friends-Cora Pettus: Served 24 clients. 0 incidents of abuse/ neglect with 5 at United Friends, 5 at Stepping Stones Group Home #1, 4 at Stepping Stones Group Home #2, 4 at Lottie-Estelle Group Home #1 and 4 at Lottle-Estelle Group Home #2. No new or amended policies/ procedures or program rules. In residential, 8 peer to peer. 1 compliant of physical abuse which was unfounded, which was later recanted and apologized to staff. In Day Support, 1 peer to peer with no major injury.

BB.  Virginia Home Health, LLC-Dwayne White: No concerns/ no abuse or neglect reported. No changes.

Upcoming Meeting Dates:

The next meeting will be October 20, 2015, 12 noon, at the La Crosse Fire Department.

CC. LHRC Human Rights Training: Virginia Freedom of Information ACT (FOIA). Mrs. Garnes provided Virginia Freedom of Information Act training to the LHRC.

ADJOURNMENT: A recommendation for a motion was made by Chairperson, Mona Rainey, to adjourn the meeting. A motion was made by Jackie Hinman to adjourn the meeting. It was seconded by Sofia Howe. All members agreed to adjourn the meeting. The meeting was adjourned at 1:47p.m.
