You must not attempt to involve yourself in any clinic activity during your placement other than agreed with the doctor unless you have organised professional cover on your own account.

Your time commitment and working pattern should be agreed both with your Trainer and other staff.

  1. As you are aware, confidentiality is essential to patient care. Please ensure that no breach of confidentiality occurs as this may result in the termination of the placement.
  1. The Trust will not normally accept any responsibility in respect of the Foundation Trust, loss or damage to your personal property and you are therefore recommended to insure yourself, as far as you think proper, against such risks.
  1. You will be subject to general policies and procedures, which apply throughout Sheffield Teaching Hospitals NHS Foundation Trust. Various policies, practices and procedures have been adopted to ensure the effective and safe operation of the Trust’s services and the welfare and interest of users of the services and those who work for it. You are expected to comply with these rules, copies of which will be available in the premises where you will be working. The Trust also has an individual Grievance Procedure and Safety at Work Policy and an Equal Opportunities Policy to which our attention is drawn.

This form must be handed to your supervising clinician upon arrival for your observation session, who will then pass this onto Debbie Liversidge, Training Co-Ordinator.

STATEMENT OF LIABILITY IN RELATION TO HONORARY CONTRACTS,

LETTERS OF AUTHORITY

Health & Safety at Work Act 1974

Under the Act Sheffield Teaching Hospital NHS Foundation Trust has a duty towards people who:

  1. Work for the Trust including casual workers, part-timers, trainees, temporary contract holders, volunteers and students.
  1. Use the workplace provided
  1. Use Sheffield Teaching Hospital NHS Foundation Trust equipment
  1. Visit the premises
  1. May be affected by Sheffield Teaching Hospital NHS Foundation Trust activities
  1. Use any products Sheffield Teaching Hospital NHS Foundation Trust make or supply
  1. Use professional services.

To off set the financial risks of accidents/incidents occurring in relation to the above the Trust carries insurance for both Public and Employee liability. Indemnity is therefore accepted in respect of legal liability for such injury or damage caused by employees or other persons with whom it contracts to supply health care services. Insurance at present excludes any cover for clinical negligence and/or breach of professional duty investing in insurance does not affect the level of liability/responsibility that Sheffield Teaching Hospital NHS Foundation Trust carries in relation to the business it undertakes.

In law the employer (Sheffield Teaching Hospital NHS Foundation Trust) is the agent assumed to control (and be responsible for) the action of its employees. This duty includes anyone to whom authority is extended to act on behalf of the Trust eg, temporary contract holders, students, volunteers. “Host” employers should have written contract of letters of authority for placements which contain the terms under which the employee is working, what he/she is required to do and how much responsibility they may assume.
