Dear InfantSEE™ Provider,
With this mailing of our InfantSEE™ Doctor Kit, you will find the following items:
- A Sample Letter to Parents – This has been developed as an example of one of the ways you can introduce InfantSEE™ to new parents. Brochures will be available to order through the AOA Order Department sometime in July.
- A Sample Letter to Pediatricians/Family Practitioners/Primary Care Doctors – Like the letter to parents, this is a tool you can use to introduce InfantSEE™ to other infant health care professionals.
- Office FAQ Sheet – Information we hope will be useful to your staff as you prepare to incorporate InfantSEE™ assessments into your practice.
Downloadable versions of these items will be available in the DoctorCenter at Like the Member area of the AOA Website, as you enter the DoctorCenter, you will be taken to a log-in screen. You will need your User Name, which is typically your AOA member number (conveniently printed on the address label of this Priority Mail envelope). Your password is your date of birth in mmddyy format. Please make it a habit to visit the InfantSEE™ web site often for program updates, and to access materials to download as they are developed.
The InfantSEE™ Forms Packet is also enclosed. These forms have been designed especially for the program and are a very important component of the assessment process. Included in this packet:
- One pad of 30 InfantSEE™ Patient History Forms – with a bound-in reorder reminder. This form should be sent to the parent/guardian of the patient to fill out prior to the scheduled appointment if possible. Please have the parent or guardian sign the form. The completed form should be filed with the patient’s records in your office. Please do not send this form to us.
- 30 2-Part InfantSEE™ Clinical Assessment Forms – to be completed during the InfantSEE™ Assessment. The top copy is for the patient’s records. The bottom copy (with the names of the patient and parent blocked out) should be sent to:

We need your help to ensure that we keep an accurate count of InfantSEE™ patients seen and to collect important diagnostic information. InfantSEE™ providers are strongly encouraged to return a copy of the Clinical Assessment Form to us immediately following every InfantSEE™ Assessment.
- A sheet of 30 pre-addressed labels are included for your convenience.
Participating providers may reorder the Forms Packet at no cost by simply calling the AOA Order Department at 800/262-2210. Brochures may be ordered through this method as well when they become available.
