MINIMUM QUALIFICATIONS REQUIREMENTS QUESTIONNAIRE
- City Licenses And Permits
Describe how the Proposer have the ability to obtain all necessary State and City licenses and Houston Airport System permits and other regulatory permits, if any, by the commencement date of the Agreement.
- Minimum Fleet Requirements
Does the Proposer have the ability to purchase (or lease on a long-term basis equivalent to no less than one (1) year) a marked dedicated fleet of at least thirty (30) 3 to 5 passenger capacity vans and five (5) 6 to 10 passenger capacity vans for IAH service; fleet of a least eighteen (18) 3 to 5 passenger capacity vans and two (2) 6 to 10 passenger capacity vans for HOU during the term of the Agreement? NOTE: Each airport should have a minimum of one wheelchair-equipped van in their fleet.
Does the Proposer agree that by completion of the Agreement’s third year, 20% of the Shared Ride Vehicles are fueled by alternative fuels, such as compressed natural gas (CNG), hybrid, or other liquefied petroleum gas (LPG)? And, to increase the vehicle percentage by at least 10 % in each of the subsequent years?
- Minimum Project Manager Experience
Does the Proposer’s Project Manager have a minimum of five (5) years’ experience in public transportation, and a preferred minimum of two (2) years’ experience in airport ground transportation? Responsible for overseeing business managing, dispatching, sales and marketing, comprehensive records, recruitment, training of employees, scheduling employees, monitoring activities, collecting and reporting data and maintaining a fleet of at least twenty five (25) vehicles?
- Minimum Annual Guarantee
Provide total guaranteed annual fee Proposer will pay the City for the first Agreement year beginning upon the date of execution of the Agreement. Does the Proposer agree to pay a minimum acceptable annual fee of at least $75,000 for IAH and $36,000 for HOU? The total guaranteed annual fee shall be provided for each year of the Agreement.

- Describe any geographic areas in addition to the Required Service Area that the Proposer proposes to serve, upon initiation and through the first year of shared ride, door-to-door passenger van service. Also include any plans for expansion of this Service Area during subsequent years and indicate the time frame that this expansion would potentially occur.
- Indicate maximum number of enroute stops that will be allowed after leaving each Airport or after picking up the first customer traveling to each Airport.
- Indicate maximum wait time at the terminal (indicate if different at each Airport) after a passenger requests transportation from a curbside coordinator until his/her designated van leaves the airport during the following time periods (may specify different wait times for initial year of operation and subsequent years of operation):
- Peak hours (define start and ending times).
- Off-peak hours (define start and ending times).
- Describe dispatching/reservations system for vehicles traveling to the Airport(s) and include the following:
- Location (or planned location) of central dispatching/reservation center.
- Hours of operation.
- Procedure for taking reservations.
- Minimum time, advance of scheduled pick-up, that customer must make reservation for travel to the Airports.
- Provision for matching passengers’ enroute to the Airports.
- Procedure for dispatching vehicles enroute to the Airports.
- Provisions for rapid reservations from all non-Airport points of origin within the Service Area.
- Procedures for accepting reservations from non-English speaking customers.
- Indicate if reservations will be taken over the internet.
- Procedure to ensure enplaning passenger arrives with adequate lead time prior to his/her flight (indicate what is considered adequate time).
- Indicate if dispatch/reservation personnel work for other companies or the same company in another location.
- Describe curbside operation/dispatching procedures for vehicles leaving the Airport and include the following:
- Procedure for dispatching vans to terminal(s) from the hold lot.
- Procedure for matching trips at the curbside.
- Describe how Proposer will maintain communication between the central dispatch system operators, the curbside coordinators, and van drivers.
- Describe any special provisions required to maintain quality operations during periodic peak periods such as during holiday seasons.
- Describe plans to accommodate the potential for future growth in the volume of customers served.
- Describe services to be provided for disabled passengers and at a minimum include the following:
- Indicate what percentage of the vehicle fleet will be accessible to disabled passengers.
- Describe measures that will be undertaken to ensure disabled passengers receive a level of service equivalent to non-disabled passengers.
- Describe plan to use stepstool or other device to assist passengers when boarding and alighting vans.
- Describe Proposer’s procedures to ensure passenger safety and include at a minimum the following:
- Schedule for vehicle safety inspections.
- Plans for monitoring van driver’s driving record throughout employment.
- Other measures that will be implemented to enhance passenger safety.
- Describe Proposer’s planned sales and marketing/advertising program, including a summary of advertising planned, by type of media (print, radio, and/or television) and any sales promotion activities planned, for the following time periods:
- Prior to and at introduction of the door-to-door, shared ride passenger van service.
- After establishment of the door-to-door, shared ride passenger van service.
- List all methods of payment that Proposer will accept and include types of credit cards.
- Describe Proposer’s procedures for responding to customer complaints received directly from customers or via HAS staff and include procedures for notifying HAS staff of their resolution.

Fares/Tariffs (Proposed)
- List proposed complete tariff including fares, charges, and terms of service. Include complete fares and charges for the Required Service Area and the Permissive Service Area with following destination areas specifically named:
- Downtown
- Midtown
- Texas Medical Center
- Galleria
- Kingwood
- Clear Lake
- List any proposed “extra charges” (e.g. extra bags) or “discounts” (e.g. round trip, multiple passengers).
- Indicate any plans for future adjustments to fares.

Equipment and Support Services
Provide the number of vehicles by size and type that Proposer plans to operate in order to provide the desired customer service. Include the following information about the vehicles:

Purchased or leased
Provisions for a destination sign
Air conditioning
Wheelchair lift equipment (specify)
Alternative fuel (specify)
Describe the Proposer’s vehicle acquisition plan and identify the proposed source(s) (including name and address) of the vehicles. If the vehicles are to be leased, provide the name and address of the leasing agency, the length of the planned lease, and a copy of a sample lease.
Indicate type of two-way radio or other communication equipment that will be installed aboard the vehicles.
Indicate type of markings or identification that will be used on the outside of the vehicles.
Indicate how many vehicles in the fleet dedicated to the Agreement are considered alternative fuel vehicles. Specify the type and source of fuel. Proposals incorporating vehicle fleets utilizing a higher percentage of alternative fuels than the minimum required amount will be evaluated accordingly.
Describe plans for temporary vehicle substitutions (e.g. vehicle out of service due to accident or maintenance).
If desired, provide other relevant information to demonstrate the quality of the vehicles and availability of an adequate number of vehicles.

Reservation/Dispatch System
Describe equipment that will be used for passenger reservations and vehicle dispatching at the reservation center and at the Airports curbsides. Indicate whether an automated shared ride dispatch and control system will be used.
Vehicle Maintenance Program
Indicate whether vehicle maintenance will be performed by Proposer’s employees or by a subcontractor.
If maintenance is to be performed by Proposer’s employees, provide the following:
Address of maintenance area/facility.
List each supervising employee and include the following:
Name.
Position for the Agreement (describe responsibilities).
Number of years’ experience at this position
Additional relevant experience.
If maintenance is to be contracted out to another party, provide the following:
Name of business.
Address of business.
Number of years that the maintenance contractor has been in business.
Length of time Proposer has used this maintenance contractor.
Name of major companies of fleet owners for which the intended maintenance contractor is currently or has recently been employed.
Describe Proposer’s vehicle maintenance schedule, including how often maintenance is scheduled for each of the following:
General upkeep (sweeping, washing interior).
Exterior cleaning.
Painting and general repairs (to retain high quality appearance).
Preventative and scheduled maintenance (e.g. engine, brakes, air conditioning).
Other (please describe).
Describe Proposer’s vehicle replacement plan and include the following:
At what age (defined as number of years past model year) will vehicles be retired?
At what mileage will vehicles be retired?

Staffing and Transition Plan
Describe Proposer’s overall staffing plans for each Airport.
List the type of personnel (i.e. project manager, deputy manager, driver, dispatcher, curbside coordinator) to be used to staff this operation and provide the following for each:
Job description (description of duties and responsibilities).
Driver’s qualifications (e.g., experience, clean DMV record, background check).
Uniform and other appearance standards.
Specialized training program (include topics that will be covered, how often the program will be offered, and proposed on-going training).
Indicate and explain any proposed use of subcontractors.
Indicate the hours that the Proposer’s managers will maintain and the location of their base of operation.
If subcontractors are to be used, provide a copy of the typical subcontract agreement indicating responsibilities of Proposer and subcontractor(s).
Describe Proposer’s plans to recruit and hire qualified employees and include procedures that will be followed to ensure that new employees meet the qualifications required for each position.
Explain how Proposer will increase staff proportionally as the volume of passengers utilizing door-to-door, shared ride passenger van service grows.
Describe the methodology for start-up of operations. The proposal shall provide a detailed schedule with its key milestones and dates each milestone shall be complete. Show the time required for hiring and training of employees, coordination with uniforms, installation of radio equipment, and airport familiarization.

Enforcement Procedures
Describe enforcement procedures to be followed if an employee or subcontractor does not comply with the rules or procedures as established by the Proposer or the City.
Describe Proposer’s training program for drivers and other employees or subcontractors concerning Airport Operating Instructions and customer service standards
Describe how the Proposer will maintain and enforce the required qualifications and appearance standards for drivers and other employees or subcontractors.
Describe how the Proposer will respond to incidents involving a driver or other employee or subcontractor disobeying posted rules and procedures or receiving a motor vehicle violation.

Qualifications of Management Personnel Dedicated to the Shared Ride Passenger Van Service
Indicate how many persons the Proposer intends to commit to the management of its service at each Airport. The Proposer must agree and stipulate that its nominated management personnel will be committed to the Agreement and will not be reassigned or replaced without the approval of the Director.
Provide adequate information to clearly demonstrate that the management personnel Proposer intends to commit to the Agreement are fully capable and qualified to perform the duties and responsibilities of the desired door-to-door, shared ride operation. For each of these management people, provide the following information:

Position to which the individual will be assigned to for the Agreement, including a description of the position, responsibilities, and airport.
Previous experience in a position similar to that which the individual will be assigned for the Agreement, including the following for each work experience:

Location where service was performed (include Airport or governmental oversight body which approved, regulated, or controlled such transportation services).
Name of company that Proposer operated (e.g., XYZ Shared Ride Passenger Vans).
Type of transportation service operated (e.g., Shared Ride Passenger Vans).
Years’ service began and ended.
Maximum number of vans operated at any one time during this period.
Number of passengers transported annually.
Specific duties for which the Proposer was responsible.
Names of Proposer’s responsible person(s) (e.g., manager, deputy manager, or equivalent).
Reference(s) including name, title, address, telephone number and email address of contact person.
List all airports or major transportation centers where Proposer operated relevant ground transportation services during the past five (5) years. Describe the type of service that was operated at each location.
List the names of all prior companies Proposer has been affiliated with, owned or operated in the State of Texas.
List all airports or major transportation centers where Proposer has had a contract agreement or an operating permit terminated either voluntarily or involuntarily. Please describe the reason for each termination.
List any prior suspensions, fines, and/or penalties levied by the City or other public agencies (including other airport operators) on the Proposer; major owners of the Proposer; or prior companies Proposer has been affiliated with, owned, or operated during the past five (5) years. Please describe the reason for each infraction and the name of the agency involved.
