1.  Highlight the problem, copy it and paste it into a new word processing document

2.  Highlight the 1st two lines and chose center align from Home: Paragraph Group.

3.  Highlight the entire problem, set your line spacer on 2.0 and set the first tab as a left aligned tab on your ruler at ½”

4.  Wherever the longest item in the first column ends measure over 1 3/8” and set a left aligned tab on your ruler (make sure everything is still highlighted).

5.  Equal out your left and right margins by adding the distance between 0 on your ruler and the first tab (1/2”) and the distance from where the longest item in the last column ends and 6½ on ruler. Each tick on your ruler represents 1/8”. Take the total and divide it by two (2) and that is where your first tab should be set. If you end up moving your first tab 3/8” (3 tick marks) left then you must also move all subsequent tabs 3/8” (3 tick marks) left so the distance between the columns remains equal. Make sure everything is highlighted to perform this step.

6.  To do the underline between the last item in the table and the Source, press tab twice, highlight the line with two tabs, go to Home: Paragraph and Tabs and set the second tab so it is 1 ½” from the first tab and under leaders choose option 4.

7.  Go to Page Layout: Page Setup, Layout, Vertical Alignment, Chose Center

8.  Insert: Header, put name at left, tab and put on the Period, tab and type the problem file name.
