JOB DESCRIPTION

JOB TITLE:Team Leader (Specialist Services)

BASE:Designated Home/Project or rotational as required

HOURS:Full Time – 37.5 hours per week (rotational)

Flexible as required

REPORTS TO:Home Manager/Deputy Home Manager

Team Co-ordinator or Designated Line Manager

Purpose of Job:

To work with the designated Line Manager to provide a high quality service for service users ensuring their holistic needs and aspirations are met.

To be responsible for the assessment of individual support needs through to the development and implementation ensuring that an evaluation of ongoing support is maintained.

To be responsible with the Line Manager for the development and management of staff.

Key Result Areas:

Operations -

1. To be responsible in conjunction with the Home Manager for the day to day operation of the home ensuring a homely/safe and happy environment for the service users is adopted and maintained and the provision of support is provided to a high standard.

2. To assist in the development, implementation and monitoring of Positive Behavioural Support plans, including leading the gathering, recording and analysis of records, the training of staff in implementation and participation in multi-disciplinary Formulation meetings.

3.To foster good relationships with service users’ relatives and within the locality, especially with the local shopkeepers, neighbours and local groups.

4.To ensure individual care programmes are provided for service users, in liaison with the Support Team.

Service Users –

  1. To understand and communicate current models of support for people with complex emotional and behavioural support needs
  1. To be responsible for and participate in the assessment of support needs ensuring its delivery is carried out and to monitor/evaluate its effectiveness.
  1. To stimulate interaction between the service users by motivating them to take part in their own development by participation in group, occupational, recreational activities and Including community activities where appropriate.
  1. To ensure that the service users obtain any prescribed drugs from their General Practitioner and that the drugs are correctly administered and stored safely.
  1. To support the development of service user involvement and empowerment in the planning and delivery of care.
  1. To enable service users to comment and complain about any aspect of their treatment or the service.
  1. To encourage the service users, if desired, to make full use of community facilities and to encourage the use of volunteers.
  1. To promote health education and well being of service user group.

Communication:

1.To assist in maintaining close liaison between medical, nursing, statutory voluntary bodies, and services within the community.

2.To liaise with members of the public, visitors and relatives visiting the home.

3.To liaise with other internal and external support staff across the Trust.

4.To attend on a regular basis and if required to facilitate Staff Team meetings and to represent the Trust/home at other meetings, both internally and externally if required.

Personal/Staff Team Development

  1. To be responsible for keeping up to date with best practice with regard to the specific needs of the client group, including Positive Behavioural Support.
  1. To fully participate and take responsibility for own development through regular ‘Job Chats’/effective learning and appraisal/performance review systems.
  1. To share responsibility with the Trust for own and professional/career development and use supervision and support of Managers as required.
  1. Provide support with the NVQ process including being an assessor or expert witness for assessing purposes.
  1. To make constructive proposals for change and improvement where appropriate.

Line Management Responsibilities –

  1. To accept delegated responsibilities of operational unit on a regular basis.
  1. To ensure the house is properly staffed during span of operational duty.
  1. When required, to lead a staff team appropriate to the registration of the Home, allocating responsibilities as required across the whole home.
  1. To ensure the staff team build good relationships with the service users, placing special importance on the method, content and appropriateness of communication involving the Home Manager as appropriate.
  1. To participate in teaching, guiding and supervision of the placement of students.
  1. To monitor staff work performance effectively, providing support and guidance where necessary and in a timely manner.
  1. To maintain good working relationships and authority within the house in accordance with Trust Policies e.g. Absence Management.
  1. To ensure all staff are aware of and adhere to the Trust’s Policies and Procedures and involve the Home Manager where appropriate.
  1. To ensure appropriate and efficient use of Home and Trust Resources involving the Home Manager where appropriate.
  1. To carry out the induction of staff members.
  1. Carry out and monitor Health and Safety checking and involving Home Manager where appropriate.

Administration and IT –

  1. To ensure electronic and manual filing systems are adopted in a logical and user friendly manner and appropriate to the Team Leaders role.
  1. To undertake specific administration tasks as directed by the Line Manager through to the production and presentation of reports if applicable, outlining the key factors and recommendations.
  1. To be aware and proficient at all record keeping, care planning, risk assessments, transport, HR, finance, person centred planning (PCP) and Health and Safety.
  1. To work within the Data Protection Act 1998 on the handling of sensitive information.

Finance –

  1. In conjunction with the Home Manager, ensure that the finance systems for the house are appropriately maintained and monitored in line with the policies issued by the Finance department and relevant legislation.
  1. To ensure that service users’ monies are monitored regularly and that their resources are spent in an appropriate manner.

Additional Responsibilities –

  1. When in charge of day to day operation take a clear lead in all matters pertaining to the home as the responsible manager, including emergency situations, service user and staff issues.
  1. Where required, update regularly and be able to demonstrate practical physical nursing/care appropriate to the service user group.
Policies and Procedures –
  1. To work with Milestones Trust’s Mission Statement, ordinary life principles, Milestones Trust policies and local operational policies.
  1. To work within Milestones Trust’s Equality & Diversity Policy.
  1. To maintain appropriate records in accordance with local/ Milestones Trust Procedures.
  1. To work within the NMC, GSCC and Trust codes of conduct.

Health and Safety –

Under the provisions contained in the Health and Safety at Work Act 1974, it is the duty of every employee to –

  1. Take reasonable care for themselves and for others at work.
  1. Take responsibility for the promotion of Health and Safety regulations as are appropriate to this area of care.
  1. Co-operate with the Trust as far as is necessary to enable them to carry out their legal duty.
  1. Not to intentionally or recklessly interfere with anything provided, including personal protective equipment for health and safety welfare at work.
