Welcome and wraparound values

The Welcome and Wraparound Values training is an overview of the Vision and Mission of Wraparound Milwaukee, as well as a historical look at how Wraparound began and has evolved into the vast and comprehensive care management entity we are today. The training offers insight into the demographics of the population we serve across the multiple programs that fall under the Wraparound Milwaukee umbrella, including Families United and the Mobile Urgent Treatment Team.

Learning objectives:

• Understand who and what Wraparound Milwaukee is.
• Understand the core values of Wraparound Milwaukee.
• Be able to explain Wraparound Milwaukee and the values to families and their supports, providers, and system partners.

Wraparound Philosophy & process: hello, help, healing, hope

The Philosophy and Process training is a unique partnership between the trainer, participants, and families who attend in a co-facilitation role. Wraparound is a process of partnering with families as the experts in their own experiences to move through hello, help, healing and hope. This process is explored in detail as Care Coordinators and families walk through Plan of Care development, beginning with hearing a family’s story. The information is then used to work collaboratively with families to identify the family vision, establish needs and benchmarks, create a strengths list, and develop strategies. Families who have been or are currently involved in Wraparound share their personal stories and experiences for others to learn from. This training occurs over the course of 2 consecutive days.

Learning objectives:

• Understand the Wraparound process and be able to explain the process to a family or team member.
• Implement the Wraparound values in practice.
• Develop and utilize engagement skills with families.
• Effectively hear a family’s story and begin to identify the vision, strengths, needs, benchmarks, and strategies.
• Understand the planning process and practice moving through the steps.
• Effectively facilitate an initial child and family POC meeting.
• Understand how to identify and engage child and family team members in the POC process.

Wraparound crisis planning

The Crisis Planning training is a continuation of the partnership with families as Care Coordinators learn to engage with families to understand what crisis is and how to both prevent and respond in a way that is meaningful and effective for the youth and families they serve. The family’s story is again used to develop the definition of crisis, identify triggers, develop prevention strategies, and create intervention strategies. The process of debriefing is explored as a learning tool that informs the proactive plan of care and the reactive crisis plan to help promote stability and healing over time.

Learning objectives:

• In partnership with a family, define crisis and write an effective crisis response plan.
• Understand the nuances of crisis planning for youth who demonstrate high risk behavior.
• Understand the connection between the proactive plan of care and reactive crisis plan to effectively reduce the likelihood of future crisis by meeting underlying safety needs.
• Understand the role of regulation and stress response in crisis planning.

Strength based documentation: needs and benchmarks

The strength based documentation: needs, and benchmarks training offers Care Coordinators an opportunity to develop the language and skills necessary to reframe problems into unmet needs and solution focused planning. This training highlights the value of context as a critical component to writing person-centered, trauma informed, respectful, and accurate plans, progress notes, court reports, and other documents. Care Coordinators will take a deeper dive into the nuances of identifying needs and creating meaningful benchmarks to assess progress toward meeting needs and realizing the family vision. This training includes an opportunity for reflection and assessment through a self-audit activity based on a plan written by the Care Coordinator.

Learning objectives:

• Demonstrate skills to write strength-based plans and progress notes.
• Understand how to use language to enhance strength based documentation.
• Understand what reframing is and how to apply it
• Determine the difference between basic needs and underlying needs.
• Develop skills to differentiate needs vs. services, goals or strategies.
• Develop meaningful and clear benchmarks to measure progress toward meeting the need.

Running Effective team meetings & conflict resolution

The running effective team meetings and conflict resolution training is an experiential training focused on skill development. This training provides Care Coordinators with information about what effective facilitation entails and offers an opportunity to practice facilitation and receive feedback through role play activities. Conflict is a normal part of the team process and offers a healthy learning and growth opportunity for teams when managed effectively. Care Coordinators have an opportunity to identify their own conflict management style and explore other styles while developing the skills needed to guide teams through conflict to a point of increased understanding and healthy resolution.

Learning objectives:

• Understand the steps of running an effective team meeting.
• Effectively perform the steps of running a child and family team meeting.
• Identify the types and root causes of conflict.
• Understand conflict resolution styles and identify personal styles of conflict management.
• Utilize techniques to manage conflict in a team setting.

Transition planning

Transition planning is critical to ensuring long-term support on the healing journey. This training teaches Care Coordinators about short and long-term planning to support families to meet their needs. Care Coordinators will explore how to strategize in a way that keeps teams moving forward in and beyond Wraparound. Transition to adulthood is a critical time in a person’s life; particular attention is paid to the supports and services available for youth aging out of the youth serving systems.

Learning objectives:

• Understand how to use the Wraparound process to facilitate good transition planning.
• Write effective transition plans and strategies that promote task shifting.
• Effectively prepare for and assist a family through transition out of Wraparound.
• Identify support programs and services available for youth aging out of care or transitioning into adulthood.

Service implementation: Provider network and finance

The Wraparound Provider network offers an array of therapeutic services that families can select from to best fit their needs and preferences. Formal providers bring a particular skillset, knowledge, or expertise to the team that can be used in a targeted way to help families meet their needs. Task shifting is the process of transferring the skills and knowledge of formal supports to the family and informal supports so that expertise is held within the family and their natural support network, and is not lost when a family transitions out of Wraparound or the provider leaves the team. The value of refinancing is explored in this training as Care Coordinators learn how to strategize in ways that promote a return on the investment in formal service provision through a reinvestment in the natural and informal supports who will be with a family long-term. This training provides a detailed description of how Wraparound is funded as a mental and behavioral health HMO, and an overview of the provider network and resource guide.

Learning objectives:

• Understand the value of having community resources active on teams.
• Identify the qualities of effective team members/providers that make them a best fit for the youth and family.
• Become familiar with the out of home care planning process.
• Develop skills to monitor and hold providers accountable in the context of the plan of care and team meeting process, including task shifting.
• Understand how to use the Resource Guide.
• Understand funding sources, Medicaid eligibility, and Social Security.

Working with schools: special education law and advocacy

The working with schools training provides Care Coordinators with an overview of special education, including the FBA, BIP, IEP, and 504 plan. Care Coordinators will learn how to advocate for children in the educational arena in a knowledgeable and effective manner to ensure the family is heard and students receive the support they need to succeed. This training includes an overview of how to support children in schools using the team process, as well as hold schools accountable regarding access to academic instruction and support in the midst of behavioral discipline practices.

Learning objectives:

• Learn to navigate and collaborate with the educational system.
• Become familiar with day treatment options and programs.
• Understand the roles, rights and responsibilities of families and schools in relation to special education, 504 plans, ADA.
• Develop a basic understanding of IEP and 504 plans.
• Understand discipline codes and rules in special education to advocate for youth with behavioral and academic challenges in school.

Understanding sexualized behavior and the commercial sexual exploitation of children (CSEC)

This training teaches Care Coordinators about the etiology of childhood sexual abuse and perpetration of sexual abuse by children. Care Coordinators will learn about risks and warning signs, safety planning, and consultation practices. Milwaukee adolescents served by Wraparound may be considered at a higher risk of becoming victims of human trafficking. This training helps Care Coordinators to understand and identify static and dynamic risk factors for CSEC, warning signs, and potential response strategies.

Learning objectives:

• Understand the prevalence and impact of childhood trauma and its relation to behavioral health.
• Understand the rationale for “high risk” policies and procedures.
• Learn how to collaborate with others to prevent child sexual abuse.
• Identify community resources to address sexual exploitation and offense specific education and treatment.
• Understand the nuances of high risk situations: youth missing from care, sexual exploitation, sexualized behavior, and suicide attempts as it relates to crisis and safety planning.

Evaluation and OUtcome measurement and assessments

This training teaches Care Coordinators about the QA/QI process as it relates to measured outcomes for families, including the agency performance review process. Care Coordinators will learn how the measured standards relate to positive outcomes for families. Care Coordinators will also learn how to read and utilize information gleaned from the assessment tools required by Wraparound, including the Youth Self Report (YSR) and Child Behavior Checklist (CBCL), as well as the YASI, a tool required by our delinquency services system partners.

Learning objectives:

• Understand the concepts of fidelity, quality assurance and evaluation and how they are connected.
• Understand how quality is monitored and its relationship to the Care Coordinator’s role.
• Become familiar with the components and the purpose of the CBCL, YSR, and YASI.
• Understand what the meaning of the T-scores and percentiles are for each individual report.
• Understand how the CBCL, YSR, and YASI are helpful in informing the conversation about the youth in clinical consultation.

Documentation: policy and procedure

This training walks Care Coordinators through the policies and procedures related to documentation of progress notes and POCs, Medicaid requirements and standards, and best practice/quality standards.

Learning objectives:

• Understand the best practice aspects of documentation as outlined in policy and procedure.
• Understand how documentation standards are integrated into the auditing process.

Mobile urgent treatment team (MUTT), medication, and diagnosis

This training provides an overview of the functions of the Wraparound Wellness clinic, including securing appointments, understanding the types of medications that may be prescribed to children, and working with families to ensure they receive consistent psychiatric care. Care Coordinators will also learn about the work that the Wellness Clinic does with youth who are pregnant to ensure the adolescents we serve who are expecting are both physically and mentally healthy. The Mobile Urgent Treatment Team works closely with Wraparound families; Care Coordinators will learn more about the role of the MUTT team and how they can collaborate regarding assessment and intervention, psychoeducation, and crisis intervention and safety planning. Care Coordinators will receive more information about the types of diagnosis, symptomology, and best practices that are frequently identified amongst the youth we serve.
