Dear Incoming Resident/Fellow:

Congratulations on your selection as a resident/fellow at UAB. As part of your training program, you will be rotating through the Birmingham VA Medical Center. The resident application materials for the Birmingham VA Medical Center can be accessed electronically at the link listed below. It will take you approximately 30-40 minutes to complete the application materials, plus 90 minutes to complete the mandatory training module.

Select the ‘New Physician Residents’ category to get started. You will see information on the forms that need to be completed as part of your application packet. Pleaseprint and follow the ‘House Staff Directions for Appointment’ checklist to ensure that you have completed all requirements for rotation. Download the forms, complete them, save the files, and print the information. The information on scheduling fingerprinting appointments and required photo identification is also included. Print the Instructions for Self-Registration for the TMS module and follow all directions carefully to complete the Mandatory Training for Trainees module. BE SURE TO MAKE A COPY of the completion certificate for the Mandatory Training for Trainees module and include it in your application packet.

Some forms require your signature. Please sign all forms as indicated to ensure that your application is processed timely. Electronically submitted forms or copied forms are not accepted. Original forms and signatures are required however; a copy of your Mandatory Training for Trainees Certificate will be accepted.

VA Application packets should be sent to your service secretary at the Birmingham VA Medical Center by May 17, 2013. Information on the names/phone numbers of service secretaries/ contacts along with the Birmingham VA Medical Center mailing address is available on the reverse side of this letter. Please call the service secretary if you have questions.
