David Oliver Beacon Award 2017

Theme: Shining Light

PURPOSE

The purpose of the David Oliver Beacon Award is for the Motor Neurone Disease Association of New Zealand to honour, acknowledge and celebrate individuals, groups or services providing a positive contribution to service, care, improvement, or innovation in management and support of motor neurone disease.

The annual award will be announced in the preceding newsletter, on the MND NZ web site, and onFacebook.

The theme of the award for that year will be announced at that time.

PROCESS

  1. The David Oliver Beacon Award theme for the year will be published three months before the event.
  2. The evaluation criteria set out the Beacon values which will be used for decision making.
  3. Nominations are to be in writing and must follow the Nomination Form layout and guidelines. Nominations are to be with MND NZ before 5pm on the closing date for the award.
  4. Nominations will be collated and circulated to the Award team (President, 2 National Council members and the National Manager) and will be scored according to the evaluation criteria
  5. The Award team will use collaborative decision processes to select the Award recipient.The Award team’s decision will be final.
  6. The Award recipient for that year will be advised within 2 months of applications closing and will be posted on the MND NZ Website andFacebook and in the next newsletter.
  7. Whenever possible, the award will be presented personally and linked to an event or a location.

Trophy or memento of recognition alongside a certificate

BEACON THEMES

The theme will be decided annually.

Themes are intended to be broad and inclusive recognising that one size does not fit all and allowing diversity of practice and innovation to be celebrated.
