Employee Benefits Summary

CCAT offers the following benefits to all employees that work 30 hours per week or more.

  1. Medical Insurance

Vision coverage is provided under theplan.

II.Dental Insurance

$1,000 maximum per person per year benefit, with an additional $1,000 for orthodontics.

III. EyeMed Vision

CCAT offers supplemental vision care benefits through EyeMed.This plan offers discounts on eye exams, eyeglasses and contact lenses. The cost varies based on members covered.

IV.Life Insurance and Accidental Death and Dismemberment

CCAT offers life insurance and AD&D with Mutual of Omaha to all employees at three times the employee’s salary, up to $450,000. This coverage has a guaranteed issue, meaning that there is no underwriting requirement for eligibility. CCAT’s contribution for Life and AD&D Insurance is 100%.

V.Long-Term Disability (LTD)

CCAT offers LTD Insurance with Mutual of Omaha to all employees at 60% of monthly earnings to a maximum of $7,000/month, to the greater of age 65 or the employee’s normal Social Security retirement age.

VI.Short- Term Disability (STD)

CCAT offers STD Insurance with Mutual of Omaha to all employees at 60% of weekly earnings to a maximum of $1,000/week for 13 weeks.

Additional Benefits:

VII.Defined Contribution Retirement Plan (401K)

  • Employees 21 years of age or older are eligible to participate in the CCAT Savings and Retirement Plan upon completing six months of service during which the employee worked at least 500 hours. Interns and temporary employees 21 years of age or older are eligible to participate upon completing one year of service during which the employee worked at least 1,000 hours. Under this Plan, CCAT’s contribution is up to 5% of an eligible employee’s salary.
  • CCAT will contribute 3% of an employee’s salary, no employee match required. The Company contribution immediately vests under a SafeHarbor plan.
  • In addition, the Employee may contribute up to the IRS maximum tax deferred limits, and CCAT will, in addition to its 3% contribution, match 50% of the employee’s contribution up to 4% of salary. This additional contribution vests incrementally over 6 years.

VIII.Flexible Spending Account

An FSA allows eligible employees to set aside a specific pretax dollar amount for unreimbursed medical, dental, and dependent care expenses. Eligibility is immediately effective upon hire.

IX.Holidays

CCAT offers ten paid holidays per year for all regular and temporary full time employees

who work 40 hours per week. CCAT’s offices are closed for seven of these holidays, with another three paid floating holidays. Regular and temporary part time

employees with employment at less than 40 hours per week are eligible for holidays on a

pro-rata basis determined by the number of weekly hours worked.

X.Paid Time Off

A.Eligibility

1.Full-Time Employees

Full-time regular and temporary employees can accrue between 13 and 20 hours of PTO per month, depending on their length of service.

2.Part-Time Employees

If you are a permanent or temporary part-time employee scheduled to work 20 or more hours per week, you receive prorated PTO accruals. Accrual is based on your full-time equivalent (FTE), or the percent of a full-time schedule that you work.

XI. Tuition Assistance Policy

Eligibility -- To be eligible for tuition assistance under this policy, employees must fulfill the following requirements:

1. Regular full-time employees with at least six (6) months of continuous employment at the time the course work begins are eligible to apply for tuition assistance benefits;

2. The course or program must improve the employee’s skills in her/his present position; and

3. Have the written approval of their Director and Human Resources.
