Statement of Faith Acknowledgement
LAUNCH OUT MINISTRIES INT’ understands that true Christians can come from many different denominational backgrounds. We attempt to create teams that are well rounded with members that vary in giftings and backgrounds in order to create fully functional “body” of believers (as outlined in reference here) to accomplish each mission. Though this is our inclination we value unity and sound doctrine above it. Therefore we ask that you read through our Statement of Faith, initial each statement and sign the bottom in acknowledge of your agreement with these foundational truths of the Christian Faith.
The programs and activities governing the form of worship of LAUNCH OUT MINISTRIES INT’, INC. shall be based upon and at all times be consistent with the following creed and beliefs:
1. We believe that all that repent of their sins and receive the Lord Jesus Christ by faith are born again of the Holy Spirit and thereby become children of God. .
2. We believe in one God, externally existing in three Persons-Father, Son, and Holy Spirit .
3. We believe that God created man in his own image; that man sinned and thereby incurred the penalty of sin which is death, physical and spiritual; that all human beings inherit a sinful nature which issues (in the case of those who reach moral responsibility) in actual transgressions involving personal guilt .
4. We believe the Bible teaches that without holiness no man can see the Lord. We believe in the doctrine of sanctification as a definite, yet progressive work of grace, commencing at the time of the new birth and continuing until the consummation of salvation .
5. We believe that the Lord Jesus Christ died for our sins, a substitutionary sacrifice according to the Scriptures and that all who believe in Him are justified on the grounds of His shed blood .
6. The ministry is open to and further truth, which the Holy Spirit may illuminate from scripture .
7. We believe that the one true Church is the whole company of those who have been redeemed by Jesus Christ and regenerated by the Holy Spirit; that the local church on earth should take its character from this conception of the Church spiritual and therefore, that the new birth and personal confession of Christ are essentials of Church membership .
8. We believe in the personal return of the Lord Jesus Christ .
9. We believe in the baptism of the Holy Spirit, empowering and equipping believers for service, with the accompanying supernatural gifts of the Holy Spirit; and in fellowship with the Holy Spirit. We believe in the divinely ordained ministries of Apostle, Evangelist, Prophet, Pastor and Teacher .
10. We believe in the resurrection of both the just and the unjust, the eternal blessedness of the redeemed, and the eternal banishment of those who have rejected the offer of salvation .
11. We believe that the Lord Jesus Christ appointed to ordinances- Baptism in water and the Lord’s Supper- to be observed as acts of obedience and as perpetual witnesses to the cardinal facts of the Christian faith; that Baptism is the immersion of the believer as a confession of identification with Christ in burial and resurrection and that the Lord’s Supper is the partaking of the emblems symbolic of the Savior’sbroken body and shed blood, in remembrance of His sacrificial death, till He comes .
12. We believe that divine healing was provided for in the Old Testament and is an integral part of the Gospel .
13. We believe in the Scriptures of the Old and New Testaments in their original writing as fully inspired of God and accept them as the supreme and final authority for faith and life. ______

Release of Liability and Assumption of Risk
In the event of being accepted for the mission trip that I am participating in under the authority of Launch Out Ministries INT’ and any of its affiliates, I here by agree as follows;
I am aware and have been informed that thiskind of ADVENTUROUS MISSIONS TRAVEL CAN BE DANGEROUS and can include certain risks and dangers, including, but not limited to, the hazards of traveling in underdeveloped areas, the hazards of traveling by any means in motorized or unmotorized transportation ( such as automobile, safari vehicle, canoe, raft, boat, train, aircraft, on foot or other unconventional means), accident or illness in remote areas without medical facilities, forces of nature, dangers of wild animals, extreme weather conditions, and physical exertion which I may not be prepared for, and evacuation difficulties, should I be disabled or injured. I accept the inherent risks of the proposed trip and acknowledge that the enjoyment of adventuring beyond normal safety of home and work is in part the reason for my participation on this trip.
I hereby waive, release, indemnify, and agree not to sue Launch Out Ministries Int’ for all or any liability to the undersigned, his/her personal representatives, next of kin, and heirs, for any or all losses, damages or injuries or any claim or demand on account of injury or emotional trauma to the person or lose of or damage to property of the undersigned, or on account of death resulting from any cause, including negligence ( but not the willful, reckless, or fraudulent conduct ) of Launch Out Ministries Int’ or others while the undersigned is participating in a trip, tour, or any travel or other arrangements made by Launch Out Ministries Int’. I also agree that I will assume the risk of and release Launch Out Ministries Int’ of all liability for any damage or injury to my body or my property or my death due to any negligent failure to obtain or administer appropriate rescue operations in the event of mishaps of injuries, including failure to obtain adequate medical services to evacuate or to supply treatment, medicine, or trained rescue personnel.
I agree that if I become ill or am injured, Launch Out Ministries Int’ or any of it affiliates may, at any cost, arrange or supply medical treatment, evacuation or any other emergency services on my behalf as Launch Out Ministries Int’believes essential for my safety and my well being. I agree that Launch Out Ministries Int’ and its affiliates are not liable or responsible for any complications that could possibly come from the administration of any emergency medical treatment.
I have read and agree to the policy on refunds and cancellations in this application packet. I am aware that should I choose not to purchase insurance coverage as advised by Launch Out Ministries Int’, I will be liable for all cost in the case of trip delays, cancellation, damage or loss of baggage and medical emergencies howsoever caused.
I accept responsibility to have all my passport, visa, vaccination and other travel documentation in proper order. I understand I may contact the embassies or consulates of the countries I plan to visit for information, and that my passport may need to be valid for at least six months after the trip ends. For information concerning any possible safety issues for international travel I know that I may call the state department at (202) 647-5525 and for information concerning vaccinations and health issues I may call the “Center for Disease Control” at (404) 332-4559.
I acknowledge and agree that this kind of ADVENTUROUS MISSIONS TRAVEL CAN BE DANGEROUS and involves serious and unpredictable risks of bodily injury, property damage and death and that the preceding release and waiver is intended to be as broad and as inclusive as permitted by law; that I am not relying on any oral or written representations of Launch Out Ministries Int’ in regards to safety, and in that I am entering this agreement by my own free will.
Any controversy or claim arising out of or relating to disagreement, or the making, performance or interpretation thereof shall be settled by binding arbitration in Orlando, FL, accordance with the rules of the American arbitration association then existing, and judgment on the arbitration award may be entered in any court having jurisdiction over the subject matter of controversy.

Terms and Conditions
Launch Out Ministries Int’, the operators of the mission, tours and/or the suppliers of services and other cooperating agencies act only in the capacity of agent for all matters relating to transportation and/or all other related travel services and assume no responsibility howsoever caused for injury, loss of or damage to person or property in connection with any service, including, but not limited to that resulting directly or indirectly from: acts of God, detention, annoyance, delays and expenses arising from quarantine, strikes, thefts, pilferage, force majeure, acts of war, insurrection, revolt or other civil uprising, other military action, failure of any means of conveyance to arrive or depart as scheduled, civil disturbance, government restrictions or regulation, discrepancies or change in transit over which it has no control. Reasonable changes in itinerary may be made at the discretion of the trip operator. On payment of deposit the depositor agrees to be bound by the above recited terms and conditions. Prices are subject to change without notice due to currency fluctuations and/or other causes.
Trip Deposit
A trip deposit is required to hold a place on any mission’s trip. The final payment is due 30 days before your departure date by money order, check or through our safe & secure online payment on our web site. If full payment is not received 30 days prior to departure Launch Out Ministries reserves the right to cancel all arrangements without refund of any deposits or any monies received in order to minimize losses.
Trip Cancellation
Although a supplementary deposit can be paid to operate a trip below our minimum participant level, Launch Out Ministries reserves the right to cancel any trip due to insufficient sign-ups. In the event of cancellation a participant will then pick another trip that suits him/her. Launch Out Ministries and participating outfitters are not responsible for any expenses incurred by trip members such as equipment, medical insurance, air tickets, etc.
Launch Out Ministries Int’ and the operators of the mission and/or suppliers of services and other cooperating agencies reserve the right to expel anyone from a mission trip without refund who acts outside of our moral code of discipline which is determined by the discretion of the trip leader, or in a manner which is unsafe for themselves or inconsiderate or unsafe to fellow travelers, and/or who ignores the requests of their guide(s) to act in a safe and considerate manner. All expenses incurred of expelling from the mission will be billed to the expelled party.
Trip Donations
All trips have a minimum donation. Launch Out Ministries is a USA registered non-profit, 501(c)(3) organization. Launch Out Ministries Int’ has complete discretion and control over the use of all donated funds. All donations are generally tax deductible. Because Launch Out Ministries Int’ maintains the required discretion and control, donations to it cannot be refunded
Insurance
A short term traveler’s insurance policy to protect the participant and his/her baggage is mandatory. Baggage is carried entirely at owner’s risk; Launch Out Ministries Int’ assumes no liability whatsoever for damage, loss or delay of property. Trip cancellation insurance will cover your losses on non-refundable air and land costs on trips cancelled due to illness.
Trip Itineraries
Launch Out Ministries International is a mission organization and not a tourist company. It’s primary commitment is to provide trip participants with an opportunity to do missions and not merely to see places. All itineraries are subject to chnage. Even if the itinerary is changed Launch Out Ministries int’ will still provide trip participants with an opportunity to do missions.
