Dear Bonitas member,

BONITAS MEDICAL FUND CHANGES FOR 2014

Bonitas recently made their 2014 benefits and contributions available and you are once again given the opportunity to change your plan option with effect from 1 January 2014. You therefore need to consider your healthcare requirements for the following year and decide if you have the most appropriate cover for your needs. You are under no obligation to change your plan option, but if you want to change your current plan option, you would need to submit your new plan option information to your Carolyn De Kock before 29 November 2013.

CHANGING YOUR PLAN OPTION

Bonitas will send you a brochure containing a detailed summary of the 2014 benefits and rates. After carefully studying the changes for 2014 you will need to decide whether you wish to change your plan option.

Please note that if you do not notify Carolyn De Kock of your plan option before the above deadline date, by completing the attached option change form, you will remain on your current option for 2014.

CONTRIBUTION INCREASES FOR 2014

The scheme announced an average weighted increase in contributions of 10.6% for 2014. The increase does vary per option.

SCHEME REBRANDING

The scheme has rebranded and adopted a new logo:

KEY HIGHLIGHTS TO THE BENEFITS ARE:

Ø  Overall Annual Limit on Primary and BonCap options removed

Ø  10% increase on the Day-to-Day benefits for Standard and Primary

Ø  Specialised dentistry co-payments on the Standard option has been reviewed

Ø  Optometry benefits on the traditional options have been increased

Ø  Non-oncology biological medication added to the BonClassic option

Ø  Lower contribution increase for the BonComprehensive option

Ø  Self Payment Gaps on BonComprehensive option have been decreased

Ø  Fourth income band introduced on the BonCap option

Ø  Income bands on BonCap option increased by an average of 5.6%

Ø  A loyalty programme will be introduced in 2014

·  Co-payments on all specialised dentistry has been removed, with the exception of orthodontic treatment

NEW LOYALTY PROGRAMME

Bonitas will be launching a loyalty programme into 2014. This programme will reward members by giving access to the following benefits;

Ø  Retail Partners / Ø  Health & Wellness
Ø  Travel & Leisure

WHEN WILL MEMBER INFORMATION SESSIONS BE HELD?

Your Alexander Forbes Health consultant or a Bonitas representative will do presentations on the 2014 plan options. You should make every effort to attend a session, even if you do not change your plan option. This will ensure that you understand the benefits that will be available to you in 2014.

GAP COVER MEDICAL INSURANCE

Xelus

Xelus is a specially designed product which provides cover for the shortfall or gap, in the event of hospitalisation involving surgery or medical treatment and for certain procedures performed on an outpatient basis.

The contribution for the mentioned policy will increase as follows:

Gap Fusion - R126.(2013) to R136 per family per month with effect from 1 January 2014.
