OFFICER TO NOTE
1.   This report should be prepared as soon as possible after an occurrence. The report is to be e-mailed to the Correctional Manager and Correctional Supervisor e-mail groups (ARG Correctional Manager and ARG Correctional Supervisor).
2.   If any force has been used upon the prisoner the Officer using that force is to report the same to the Correctional Manager as soon as practicable after the event.
3.   When reporting the facts in the space provided, you are to state clearly what you observed. If the space provided is insufficient to record the facts, attach another sheet to the report. Where any conversation took place, or where any words were used, the actual conversation or words used, are to be quoted in the first person (i.e. I said, “…..” The prisoner stated “…..”.)
4.   Leave a line between each paragraph.

REPORT

On Monday, 20 March 2017 I was rostered on duty from 0630 hours until 1730 hours.

At approximately:
1613 I heard 'ALERT CODE YELLOW NON COMPLIANT PRISONER'. As I was on my way to A1 another 'ALERT CODE YELLOW, OFFICER NEEDS ASSISTANCE- STAFF ASSAULT' was called.
On arrival Prisoner E54258 SMITH, Kane was on the floor being restrained on the airlock floor. I entered the unit with responding staff and saw CSO E ADAMS directing Prisoner D72701 MARKWICK, Wayne to the yard, to which he appeared to be refusing. I approached and directed Prisoner MARKWICK to get into the exercise yard. He was refusing so I pushed him back. He was then joined by another prisoner who I am lead to believe is Prisoner A68338 UTRATNY, Rory. Both prisoners were again directed to go into the yard, they were refusing to go so I pushed them backwards into the yard where myself and other officers secured the yard door.
I then saw a prisoner near the Television area with multilple officers attempting to gain compliance. Prisoner C17810 DACOSTA, Hugo was being compliant with one officer. I helped to keep him compliant on the window.
Once Prisoner DACOSTA had enough staff, I assisted CSO D Lloyd with restraining prisoner D58109 MANWARRING, Travis near the phone area.
Compliance was gained with the unit and Prisoner DACOSTA was escorted out to medical with officers.
I then escorted CSO WILLIAMS and MOSSMAN to medical for assessment. On returning to the unit I assisted with the unit lockdown of the remaining prisoners.
Once the unit was secured and the alert stood down, I escorted CSO HART to medical for assessment.
