ARTHUR GORRIE CORRECTIONAL CENTRE
OFFICER TO NOTE
1.   This report should be prepared as soon as possible after an occurrence. The report is to be e-mailed to the Correctional Manager and Correctional Supervisor e-mail groups (ARG Correctional Manager and ARG Correctional Supervisor).
2.   If any force has been used upon the prisoner the Officer using that force is to report the same to the Correctional Manager as soon as practicable after the event.
3.   When reporting the facts in the space provided, you are to state clearly what you observed. If the space provided is insufficient to record the facts, attach another sheet to the report. Where any conversation took place, or where any words were used, the actual conversation or words used, are to be quoted in the first person (i.e. I said, “…..” The prisoner stated “…..”.)
4.   Leave a line between each paragraph.

REPORT

On Monday, 20 March 2017 I was rostered on duty from 0630 hours until 1730 hours.

At approximately:
1613 hours an Alert Code Yellow Officer Requires Assistance was called in A1.
Shortly after I was advised to attend the Centre Emergency Preparation Room and assemble a team of eight Officers and stand by for further instructions.
I was then contacted by CS GOTT and advised to kit up a team of three in personal protective equipment and deploy the rest of the Officers to assist with a lockdown of the A Side.
I have then assisted CSO AJEZ, CSO TIMAI and CSO LANE to don their equipment.
CS GOTT has been advised that the team was ready and he has advised that they would be required to meet him outside the B-Side Movement.
CSO MURRAY has escorted the Team to the Stores and I have then attended the B-Side of the Prison to assist with Unit Lock Downs.
Once completed I have returned to my normal duties.
