AGREEMENT FOR STUDENTS BORROWING COMPUTER/TECHNOLOGY EQUIPMENT

1.As student in the Baltimore City Public School System, I, the undersigned, am borrowing the following equipment beginning on ___ __ and I understand and agree to the terms and conditions set forth in this Agreement for the use of the computer and equipment:

2.Each Baltimore City Public School System (City Schools) student who borrows computer equipment has a duty to take all reasonable care to protect the equipment against any damage, loss or theft, normal wear and tear expected.

3.If the computer equipment is damaged, lost or stolen, the student will immediately report this to the local authorities (in the event of theft or malicious destruction). A copy of the police report should be sent to the Information Technology Department (ITD)Helpdesk at 200 E. North Ave Baltimore, MD 21202 Room 001 (fax. 410-837-1838) ATTN – ITD Helpdesk no later than 24hours after discovery of loss.

4.In the event that the computer equipment is damagedthrough negligence, the student will be responsible for the repair costs. The Baltimore City Public School System will be responsible for normal maintenance of the computer equipment.

5.Each student agrees that he/she is responsible for testing and completely checking the condition of the computer equipment prior to borrowing the equipment and reporting any existing damage to the computer equipment to the appropriate authority at that time. Each student will be responsible for the computer equipment's condition upon return except for damage reported prior to borrowing.

6.Each student agrees to use the computer equipment at his/her own risk and agrees to hold harmless and absolve from all liability the Board of School Commissioners and its departments, students and agents for any damage to persons or property caused by any malfunction of the computer equipment or failure to properly operate the equipment.

7.Each student agrees that, if his/herparenthas purchased renter's or homeowner's insurance policies, his/her parent will schedule the computer equipment as personal property if required by the insurer for coverage.

8.The computer equipment borrowed is the property of the Board of School Commissioners and the Baltimore City Public School System, Department of Education. The computer equipment will be used by the student for work related business only.

9.The student will return the equipment at the designated time in the same condition as when it was borrowed to the appropriate authority. In the event that the student's stops matriculating prior to the designated return date, the computer equipment will be returned immediately upon notice by the studentof withdrawal or the school district of expulsion. The lending authority has the right at any time to require the return of the computer equipment.
