Admissions: Relationship Component for Under 18 Students

OVERVIEW

Students enrolling in classes while under the age of 18-years must have a parent or legal guardian acknowledge and agree or disagree to text, robo-calling, or additional telephone communications from Maricopa Community Colleges. In addition, the parent or legal guardian must acknowledge and agree to the Financial Agreement indicating they understand and agree to financial responsibility for all tuition and fees associated with the students account.

This consent will be received either by Electronic or Paper form- “Financial Responsibility and Consent for Students Under the Age of 18”. Information from that form will be used to complete the Relationship Component.

This form will be required every term until the student turns 18 OR until the parent signs the one-time form that will be put in place August 1, 2017.

SCOPE

This process will performed by the appropriate Admissions, Records, and Registration employee, as well as additional employees working in areas such as Dual Enrollment, Recruiting / Outreach, or similar areas of Student Services and Support.

OBJECTIVES

By the end of this document, you will be able to:

  • Access the Relationship Component for a student,
  • Add the parent(s) or legal guardian(s) address and contact information, and
  • Enter the appropriate acknowledgements required from the parent(s) or legal guardian(s).

SCENARIO

Martha B. is hand entering student information for the Dual Enrollment department who has just returned from an event at a nearby high school. The documents include parent / legal guardian information that Martha will need to include on each students account to ensure they meet all necessary requirements before they can proceed with enrolling in classes.

Before you begin…
The Relationship Component serves to address a requirement for all students who are minors under the age of 18-years; the acknowledgements and disclosures included in this componentmust be agreed to by the appropriate parent or legal guardian prior to the student being enrolled in class(es). A service indicator (U18) will hold registration until this is complete. Completion of the relationship component will trigger the release of the U18 hold. This process runs hourly to remove the holds.
Students can complete the form two ways; via e form which they can access by clicking on the hold in their student center, or by completing the paper version of the form.
If you have additional questions or concerns, please consult the MCCCD Policy, or the appropriate Department or Administration at your college.
Step 1 / Navigate to the Relationships
Select Main MenuCampus Community > Personal Information > Biographical > Relationships > Relationships
Choose Add New Value tab and enter the appropriateStudent ID number.
Click the Add button. /
Step 2 / Relationship – Basic Information
On the Relationship tab, the Effective Date field will default to today. You will need to enter Status, Relationship, and Name of the individual. Name field uses “Last, First” convention.
Next, enter the NID Country and NID Type. For NID Type, you may choose fromIndividual Taxpayer ID (ITIN) or Social Security Number(SSN). Enter either the Taxpayer IDor the SSN number that has been provided on the form into the National ID field. None of the other fields on this page will be filled out.
When ready, click the blue MCCCD Attributes link located to the top right of the page. /
Step 3 / Relationship – Opt In / Acknowledgements
You will be taken to a separate page forMCCCD Relationship Attributes.
Enter the first field with the appropriate Term. This should be the term the form was completed.TheSMSRobo-Callingand FinancialAgreement buttons will default to “Yes.” Based upon how the SMSRobo-Calling field is answered on the form, this field can be changed to “No.”
If the student has completed the One time Form (as indicated on the form version) select “Yes” for that field. If they have completed a term specific form, select “No”. For students that select “No”, a new form will be sent to them prior to enrollment next term.
Note: The Financial Agreementacknowledgement is a requirement for all students under the age of 18. If the form indicates otherwise, the Relationship Component should not be completed and saved at this time. Consult the appropriate individual or department at your college regarding this issue and how to proceed.
Click the OK button. /
Step 4 / Relationship – Physical Address
You will be returned to the Relationships page.
Next, click on the Relationship Address tab. From here, click on a blue Edit Address link in the middle of the page to the right.
You will be taken to Edit Address page. Enter the address information provided on the form into the corresponding fields on the page.
Click the OK button. /

Step 5 / Relationship - Email Address
You will be returned to theRelationship Address tab.
On the lower portion of the page, you will see the field for Email Address. Enter the email address that has been provided on the form.
Next, click on the Relationship Detail tab. /
Step 6 / Relationship – Telephone Number
Next, you can enter any or all telephone numbers for Day, Eve, and Fax that have been provided on the form.
When ready, click on the Relationships tab. /
Step 7 / Save Information
At this time, all necessary fields have been completed. Click the Save button to complete the process. /
