This certifies that ______and ______is/are the owner(s) of the right of sepulture in GARDEN OF______, BLOCK_____, LOT_____, SPACE______, in Killeen Memorial Park, as per plat thereof on file in the above said office of said Killeen Memorial Park, which said lot, crypt or niche is to be used for cemetery purposes only, that is, for the sole purpose of sepulture of the dead of the human race, subject to the Dedication of said Killeen Memorial Park, recorded in the official records of Bell County, Texas, and the Rules and Regulations (and/or any amendments thereto) of said Killeen Memorial Park maintained at the said Cemetery office.

Killeen Memorial Park agrees to provide maintenance care, as defined in the Dedication and herein, without further charge. This cemetery is operated as a perpetual care cemetery, which means that a perpetual care fund for its maintenance has been established in conformity with the laws of the State of Texas. Perpetual care means to maintain, repair and care for the cemetery, including the roads on the cemetery property.

NOTICE: This certificate, and the rights of sepulture herein referenced, shall not be valid or effective until this Certificate has been executed by an authorized Registrar for the Cemetery, located at the cemetery, in the space provided below.

IN TESTIMONY WHEREOF, Central Texas-Killeen Memorial Park Inc., a Texas Corporation, the owner of Killeen Memorial Park Cemetery, has caused this instrument to be signed by its President or Vice-President, and its Secretary or Assistant Secretary and its corporate seal to be affixed, this______day of ______, 20___.

VALIDATION BY CEMETERY REGISTRAR

In my capacity as an authorized Cemetery Registrar of Killeen Memorial Park located at the Cemetery, I hereby acknowledge and confirm that the property described herein has been fully paid for by, or on behalf of, owner, and that all purchaser’s obligations have been met. I further acknowledge, in my said capacity, that this Certificate has been, and is now, hereby issued and delivered to, or for the benefit of, the said owner, in accordance with the applicable Rules and Regulations of the Cemetery.
