4.In the “Teaching, Research & Social Service” collumn, external applicants for assistant & associate professorship are supposed to include information within the last three years, and applicants for full professorship are supposed to include information within the last five years. E.g., if you apply in August 2016, the last three years start from January 2013, and the last five years start from January 2011. Internal applicants are supposed to include achievements at your current position.

5. Pages may be added to show all the necessary information.

6. Page 7 to 9are supposed to be filled in by the relevant college/school and the University.
