Reports to: Integrated Care Service Manager

Place of work: 67 High Street, Thornbury and at other locations as required

Hours of work: 16 hours per week (to be worked flexibly as agreed with the Line Manager)

Salary: £17,139 fte (£7,312actual)

plus up to 7% pension after qualifying period

Holiday: 25 days annual leave and public holidays

(Both calculated pro rata for part-time roles)

Tenure: Fixed term contract to 31 March 2019(There is the possibility that this term may be extended depending on the outcome of the pilot.)

Probationary Period:6 months

Purpose of the Job

  • Work effectively within the Integrated Care Service to ensure older people with long term conditions who are risk of unplanned hospital admissionare supported to live the lives they wish to, to the best of their abilities, keeping their lifestyle goals at the forefront of their care planning.
  • Provide administrative support to the Integrated Care Practitioners
  • Contribute to the one-to-one follow up to service usersthat will empower them to act more independently and achieve their self-defined goals

This may involve supporting people to identify and attend local clubs and enabling them to join in with different community activities, and/or supporting them with their hobbies and interests, whilst they build their confidence and self-esteem.

The nature of this post may include travel therefore a requirement will be access to and the use of a reliable vehicle. Car mileage will be paid at the prevailing HMRC approved rate (currently 45p per mile).

Key Responsibilities:

  • To provide individual, bespoke, motivational support to older people in order for them to achievetheir lifestyle goals, recognising that support may be for varying periods of time and maybe offered on a 1:1 basis or through supporting a group of people to meet up and enjoy each other’s company.
  • To inspire and motivate people to make healthier lifestyle choices by offering options such as physical activities, healthy eating and long term condition peer support into their action plans.
  • Working with Integrated Care Practitioners to help service users understand what triggers a crisis situation and what response is required, by whom and when to avert future crisis situations, informing their Integrated Care Plans and liaising with their Integrated Care Practitioners.
  • Contribute to the administrative support required by the Integrated Care team
  • Keep accurate and up-to-date records of visits, meetings and referrals made on behalf of the client and ensure AUKSG’s database is kept up to date.
  • Explore using local community networks and create opportunities to help the person to build their own new or existing sustainable social networks, offering choices that support their action planning and goal attainment.
  • Understand and maintain a good working knowledge of the range of support services on offer e.g. assistive technology, equipment, specialist services e.g. hydration, dementia care, police, social care, therapy, handyperson, health trainers, peer support groups.
  • This post will involve lone working and visiting people in their own homes including exposure to dealing with difficult and emotional circumstances/situations.
  • To enable older people to increase their confidence and independence.
  • To work within all policies and procedures relevant to AUKSG and the Integrated Care Serviceand ensure that client information remains confidential.
  • To undertake other duties that are consistent with the duties and responsibilities of this type of post.

Essential Criteria

  • Experience of working with older people, with maturity and sensitivity
  • Knowledge of the issues older people face to include health and non-health needs
  • A good level of administrative and personal organisational skills
  • Excellent verbal, written communication, listening skills and motivational skills
  • Ability to work as part of a team
  • Willingness and ability to develop new ways of working where required
  • Ability to use initiative and work independently, taking responsibility for problems that arise
  • Ability to help people to help themselves and not create a dependency
  • Commitment to the aims and objectives of AUKSG
  • Commitment to promoting and supporting equal opportunities and diversity

Desirable Criteria

  • Experience of relevant work in the health and social care sector
  • Experience of working with older people affected by dementia
