HMHM / Home and Mobile Health Monitoring - COPD

Home and mobile health monitoring has been implemented in South Ayrshire for 6 years. The main hub is based within Biggart Hospital and is part of the Intermediate Care Team (ICT). Practices refer patients who have had an exacerbation of COPD to the single point of contact where arrangements are put in place for them to monitor their condition from home using a Telehealth POD.
Home and Mobile Health Monitoring – Chronic Heart Failure

Home and mobile health monitoring has been set up within the Heart Failure Specialist Nurse Service for 6 years, referrals are sent to the main hub in South Ayrshire from the specialist nurses after an initial assessment and if the patient is deemed to be suitable for the service.

FLO Cognitive Behavioural Therapy

14.30 / Beating the Blues is an online treatment programme that uses cognitive behavioural therapy (CBT) to help people experiencing depression and/or anxiety. It consists of eight, one hour sessions completed weekly either in your home or at a community location.
TEC Diabetes

Diabetes is a long-term and progressive condition that can potentially have devastating effects on health. However, evidence shows that many of the complications associated with diabetes can be prevented or delayed through a combination of clinical management, patient self-management and the appropriate use of pharmaceuticals.
There has been an upward trend in the prevalence of diabetes and it is likely that this will continue for the foreseeable future
FLO Primary Care COPD Pathway

The FLO Primary Care pathway is primarily aimed at newly diagnosed COPD Patients. The aim of the pathway is to learn patients to self manage their condition and to have an understanding of when they are likely to be going into exacerbation and how to deal with this using medications in reserve.
