Given to an individual who has contributed significantly to the Coalition’s mission of working in partnership to achieve permanent solutions to prevent, reduce, and end homelessness.
 / Outstanding Service Provider – Agency
Given to an agency or organization that has contributed significantly to the Coalition’s mission of working in partnership to achieve permanent solutions to prevent, reduce, and end homelessness.
 / Excellence in Housing Award
Given to a property owner or manager who has contributed significantly to advancing affordable housing in Fargo-Moorhead. (Nominated by LRMF Advisory Board.)
 / Community Champion in Ending Homelessness
Given to an individual or organization whose vision for change, social justice, and upholding homeless human rights has impacted the work of preventing, reducing, and ending Homelessness in Fargo-Moorhead.
 / Most Valuable Player in Ending Homelessness
Given to an individual who has an unflagging dedication to improving homeless services in Fargo-Moorhead.
 / Rookie of the Year Award
Given to an individual service provider with less than two years of experience who has exhibited excellence in the field and shows promise for continued future work in ending homelessness.
General Criteria Used by the Board of Directors:

Excellence in the category nominated.
Commitment to Housing First philosophy.
Commitment to the mission and work of the FM Coalition.

Nominee has not received the award in the immediate previous year.
Nominations must be by a party other than the nominee.
Board reserves the right to not present awards in any category.

Keep your statement to around 300 words, keeping the box below from starting a new page. (If your statement goes onto a second page, you have gone over the limit!
