WAIVER/RELEASE FORM

STATEMENT OF LIABILITY AND SPORTSMANSHIP

All participants are required to assume all risks by signing this waiver/release form. Each player must sign their name below. Although a player may have player in Synagogue Softball prior, each players must sign a new waiver/release form each season.

By signing this form, I understand that the risks I assume by participating in this L.A. City Sports Program, named Synagogue Softball (“League”) includes but is not limited to serious head and other bodily injuries. In consideration of receiving permission to participate in this League, I hereby release the City of Los Angeles, Department of Parks and Recreation, Synagogue Softball, it’s officer and agents, all Temples and Synagogues participating, from all liabilities for any personal injury, death, damage or loss of any kind that I may experience arising from or relating to my participating in this League. No oral representations, statements or inducements apart from this written waiver have been made. My decision to participate in this League is voluntarily and I assume all risks or loss, damage or injury (including death), that may occur through my participation in this League.

I understand that by signing this waiver/release, I am giving up the right for myself and/or any family heirs to sue the City of Los Angeles, Synagogue Softball, its officers and agents, its sponsors and affiliates and all participating Temples for any injury, loss, damage, or liability resulting from my participating in this League.

Furthermore, I understand and elect to participate in this League and agreed to adhere to the league’s rules, especially to the rules relating to sportsmanship. I agree to maintain a cooperative attitude towards my opponents and will refrain from using foul language. I will not act in a violent manner towards a player or umpire. I recognize the responsibility and penalty if I violate the league’s sportsmanship pledge. I will respect the women and children in attendance as well as all those involved in the game. I agree to abide by all the Rules and Regulations governing this League.

I acknowledge that I have read the above and that I understand each and every one of the provisions in this waiver/release of liability and indemnification agreement and sportsmanship statement and agree to abide by them.
