Job Description

Job Purpose

To be an effective part of the clinical team to ensure the provision of a high level of specialist palliative care within a multi disciplinary approach.

Key Tasks

1. To assist patients with their activities of daily living, under the direction and supervision of professional nursing staff. To escort patients as necessary when moving outside their usual care setting

2. To participate in agreed programmes of care ensuring that all care given is clearly and accurately documented in line with standards and seeking support from senior staff as necessary.

3. To welcome all patient’s carers, visitors and family members in a courteous and friendly manner, providing additional support as required.

4. To follow Saint Michael’s procedures in caring for patients after death and providing support to family and carers as necessary

5. To support new members of the Health Care Team in their orientation process.

6. To communicate with members of the team, patients and carers clearly and effectively, seeking help as needed.

7. To maintain high standards of comfort and hygiene and meet the needs of patients work with the Housekeeping and Catering teams in dealing with tasks such as laundry, preparing refreshments and serving meals.

8. To work closely with the Multi-disciplinary Team andvolunteers,promoting supportive working relationships.

9. To follow Saint Michael’s policies and practice in taking care of patients property and valuables.

10. To maintain a safe working environment at all times, making the best and most economic use of resources, and equipment. To order new stores as required in line with procedures.

11. To ensure that all aspects of confidentiality are maintained in line with Saint Michael’s policy and the principles of information governance.

To undertake all duties in accordance with the law, hospice philosophy, recognised professional guidelines and the hospice commitment to equality of opportunity and collective responsibility.

The above tasks are indicative of the role to be carried out; they may change from time to time in collaboration with the post holder and in line with service need.

Essential Education and Qualifications
Essential Experience / Previous experience of a palliative care environment
Essential Knowledge Skill / Good communication and interpersonal skiils
Understanding of patient care.
Patient centred approach
An understanding of and commitment to multi disciplinary working
Understanding of patient protection from abuse
Team working skills
Understanding of infection prevention and control
Understanding of health and safety
Understanding of the importance of accurate recording and record keeping
Understanding of working with volunteers
