Required Documentation
Below is a listing of documents/informationrequiredand acceptedby the Early Learning Coalition of Okaloosa/Walton Counties for eligibility determination.
Verification of All Family Earned Income for household members included in your family size.
Four (4) weekly, Two (2) biweekly, Two (2) semi-monthly, One (1) monthly of current, consecutive pay stubs – Must have company name, parent/guardian name, and required (at least 20 hours per week) #of hours worked(if your pay stubs do not have # of hours then a job verification form or a letter from your employer will be required).
Complete currentincome verification form- Completed and signed by your employer
Self-employment verification/Casual Labor- IRS Tax Schedule C,1099’s, accounting records, contracts, receipts, Work Calendar showing dates worked, for whom, amount received. Expenses must be verifiable with receipts of payment (if you are paid by someone else, you may use a job verification form).
*Any and all earned income and working hours must be verifiable.*
Verification of all other income including, but not limited to:
Child Support – Clerk of Court website, statement from non-custodial parent with amount and frequency of pay. Parent may self attest during school readiness appointment.
Alimony – Court order
Social Security – Current Award letter (updated in January)
Unemployment - Unemployment Compensation print out
Relative Caregiver/TANF– Documentation by DCF or current award letter
School Schedule or a letterfrom an accredited school/institute indicating:
Number of classroom hours & lab hours (Fall: 12 credit hours = FT/Summer: 9 credit hours = FT)
Date the semester/training starts and ends
Current Florida Drivers License or Government issued Picture ID (for both parents if 2 parent household)
Date of Birth/Citizenship - Verification of all children in the household.

US Passport
Lawfully admitted Alien documentation (I-94, I-94A, I-97, I-551, I-766)
Certificate of US citizenship or naturalization
Proof of Residency * Must be dated within the last 12 months*
Current Driver’s License,Current Pay stub, Voter’s Registration Card
Current Utility Bill – electric, water, cable, landline telephone (must show service address)
Current Residential rental agreement – signed by landlord & tenant
Notorized statement from parent stating the current address as well as a letter from the landlord or home owner that verifies the client lives at the same address.
Proof of Legal Guardianship (if applicable)
Court documents indicating guardianship
Proof of Participation in Relative caregiver program
Award letter of Payee status of SS benefits
Notarized statement from parent(s) – Statement requirements - Anticipated time frame, parent’s name, address, telephone #, and another form of corroborating evidence such as: filing for child support or verification of the parent’s absence (incarceration or treatment facility)
Current Food Stamp Verification showing benefit amount and household members
Current Housing Assistance (Section 8) letter or HUD letter
Social Security Cards (optional)
Before your appointment, please be advised that you will need to select and secure availability with your chosen provider
These documents are required to complete your eligibility process. Failure to submit required documents could result in the termination or denial of your School Readiness services. If you are unable to attend your appointment, you must notify the coalition 24 hours in advance to reschedule.
